Postmenapozal kadınlarda hormon replasman tedavisinin kuru göz gelişimine ve konjonktiva sitolojisine etkileri

Özet
AMAÇ: Menapoz sonrası hormon replasman tedavisinin kuru göz gelişimi ve konjonktiva sitolojisi üzerindeki etkilerini incelemek. GEREÇ VE YÖNTEM: Hormon replasman tedavisi (HRT) alan ve almayan postmenapozal 40 kadın ve kontrol grubu olarak premenapozal 20 kadın (toplam 120 göz) çalışmaya katıldı. Hastalara Schirmer testi yapıldı ve floresein kırılma zamanları (FKZ) ölçüldü. Konjonktiva sitolojisi impresyon tekniği ile yapıldı ve Nelson kriterlerine göre sınıflandırıldı. BULGULAR: Schirmer testi sonuçlarına göre, HRT kullanan hastalar ile kullanmayan ve kontrol grubu arasında anlamlı bir fark bulunmadı. Ancak HRT kullanmayan grubun Schirmer ölçümlerinin kontrol grubuna göre anlamlı olarak düşük olduğu izlendi (p<0.05). FKZ testi sonuçları incelendiğinde HRT kullanan ve kullanmayan hasta grupları arasında anlamlı fark görülmezken, HRT kullanan ve kullanmayanların kırılma zamanları kontrol grubu ile ayrı ayrı karşılaştırıldığında anlamlı olarak kısa olduğu görüldü (p<0.05). İmpresyon sitolojileri değerlendirildiğinde her üç grubun da birbirinden anlamlı derecede farklı olduğu (p<0.05) HRT kullanmayan grupta daha fazla skuamöz metaplazi olduğu görüldü. SONUÇ: Menapoz sonrası kadınlarda menapoz öncesi döneme göre kuru göz gelişme riski daha fazladır. Menapoz sonrası HRT kullanımı göz yaşı miktarını etkilemezken konjonktivada gelişebilecek skuamöz metaplaziyi azaltmaktadır.
Abstract
PURPOSE: To evaluate the effect of hormone replacement therapy (HRT) on development of dry eye and conjunctival cytology in women after menopause. MATERIALS AND METHODS: Forty postmenopausal women who were either using HRT or not and 20 premenopausal women as control group (120 eyes as total) were included in this study. Schirmer tests were done and flourescein break up times (BUT) were measured. Conjunctival cytology was evaluated by using impression cytology technique and classified according to Nelson's criteria. RESULTS: According to Schirmer test results, women using HRT did not significantly differ from those not using HRT or control. But women not using HRT had significantly lower values compared to control group (p<0.05). When flourescein BUT results were evaluated, there was no significant difference between women using HRT and not using HRT; but when HRT using and not using groups were compared to control group, each one had significantly shorter BUTs (p<0.05). When impression cytology grades were evaluated, all 3 groups were significantly different from each other (p<0.05), such that women not using HRT showed significantly more squamous metaplasia. CONCLUSION: Women have a higher risk of developing dry eye after menopause compared to premenopausal period. Using HRT after menopause does not effect the amount of tear, rather decreases the conjunctival squamous metaplasia.
