Özet

Amaç: Erişkin ve pediatrik dönemde orbita tümörü tanısı alan olguların histopatolojik dağılımlarını vurgulamaktır. Gereç ve Yöntem: 1992-2002 yılları arasında orbita tümörü tanısı alan 124 erişkin (71 erkek, 53 kadın) ve 67 pediatrik (25 erkek, 42 kadın) toplam 191 olgunun başvuru yaşı, başvuru şikayeti ve histopatolojik tanıları retrospektif olarak incelendi. Uyum sağlayan tüm olgularda görme keskinlikleri, göz içi basıncı, ön ve arka segment muayenesi yapıldı, göz dışı kas fonksiyonları Hess perdesi, proptozis düzeyi ise Hertel ekzoftalmometre ile değerlendirildi. Olguların radyolojik incelemelerinde bilgisayarlı tomografi veya manyetik rezonans görüntüleme yöntemleri kullanıldı. Bulgular: Erişkin olguların tanı anında yaş ortalaması 41.5 yıl olarak bulundu. Başvuru şikayeti, 82 olguda gözde çıkıklık fark edilmesi, 21 olguda baş veya göz ağrısı, 13 olguda görmede azalma veya çift görmeydi, 8 olguda ise tanı rutin göz muayenesi sırasında rastlantısal olarak konuldu. Histopatolojik incelemede 46 olguda kistik, 16 olguda ikincil, 14 olguda inflamatuar, 13 olguda vasküler, 11 olguda lakrimal bez, 10 olguda hematopoetik, 6 olguda nörojenik, 5 olguda fibrokonnektif doku, 3 olguda ise metastatik orbita tümörü tespit edildi. Pediatrik olguların tanı anında ortalama yaşı 8.8 yıl olarak bulundu. Başvuru şikayeti; 61 olguda gözde çıkıklık fark edilmesi, 3 olguda şaşılık, 3 olguda ise görmede azalmaydı. Histopatolojik incelemede 31 olguya kistik, 12 olguya çizgili kas, 11 olguya vasküler, 6 olguya hematopoetik, 3 olguya nörojenik, 2 olguya histiyositik, 1 olguya inflamatuar, 1 olguya ise fibrokonnektif orbita tümörü tanısı konuldu. Sonuç: Erişkin ve pediatrik yaş grubunda en sık rastlanan orbita tümörü kistik tümörlerdir ve en sık başvuru semptomu proptozistir. Elde edilen tümör dağılımı sonuçları, daha geniş çaplı çalışmaların sonuçlarıyla paralellik göstermektedir.

Abstract

Purpose: To evaluate the histopathological distribution of adult and pediatric onset orbital tumors. Materials and Methods: During the period of 1992-2002, 124 adult (71 male, 53 female) and 67 pediatric (25 male, 42 female), totally 191 cases with orbital tumors were retrospectively evaluated according to their age, presenting symptoms and histopathological diagnosis. Visual acuity, intraocular pressure, anterior and posterior segment examinations were performed in all cases who co-operated. Extraocular muscle functions were assessed with Hess chart and the amount of proptosis was examined with Hertel exophthalmometer. Computerized tomography and magnetic resonance imaging were used for radiological evaluation of the cases. Results: In adult cases, the mean age was 41.5 years. The presenting symptoms were proptosis in 82 cases, headache and periorbital pain in 21 cases, decrease of visual acuity or diplopia in 13 cases. Eight cases were diagnosed accidentally during routine ophthalmological examination. The histopathological diagnosis was cystic tumors in 46 cases, secondary tumors in 16 cases, inflammatory tumors in 14 cases, vascular tumors in 13 cases, lacrimal gland tumors in 11 cases, hematopoetic tumors in 10 cases, neurogenic tumors in 6 cases, fibroconnective tumors in 5 cases and metastatic tumors in 3 cases. In pediatric cases, the mean age was 8.8 years. The presenting symptoms were proptosis in 61 cases, diplopia in 3 cases and decrease of visual acuity in 3 cases. The histopathological diagnosis was cystic tumors in 31 cases, striated muscle tumor in 12 cases, vascular tumors in 11 cases, hematopoetic tumors in 6 cases, neurogenic tumors in 3 cases, histiocytic tumors in 2 cases inflammatory tumor in 1 case and fibroconnective tumor in 1 case. Conclusion: Cystic tumors are the most common orbital tumors in both adult and pediatric cases and the most common presenting symptom is proptosis. These results are in concordance with other larger series evaluating the histopathological distribution of orbital tumors.
