Göz içi lens dislokasyonlarında cerrahi tedavi yöntemleri ve sonuçları

Özet
Amaç: Katarakt cerrahisinden sonra gelişen göz içi lens (GİL) dislokasyonlarında, cerra- hi yaklaşımlar, komplikasyonlar ve görsel sonuçların değerlendirilmesidir. Gereç ve Yöntem: 2008-2012 yılları arasında kliniğimizde tedavi edilen GİL dislokasyon olguları retrospektif olarak incelendi. Olgular, preoperatif ve postoperatif görme keskinlikleri (GK), uygulanan cerrahi teknikler, eşlik eden oküler patolojiler, intraoperatif ve postoperatif komplikasyonlar ve takip süreleri açısından değerlendirildi. Bulgular: GİL dislokasyonu olan 31 olgunun 31 gözü çalışmaya dahil edildi. Olguların 23'ünde (%74) GİL subluksasyonu, 8'inde (%26) GİL luksasyonu mevcuttu. Olguların ortala- ma yaş ve takip süreleri sırasıyla 56,7±20,8 yıl (17-84 yıl) ve 7,9±6,5 ay (2-24 ay) idi. Preope- ratif GK ortalama 1,59±1,01 logMAR (el hareketi-0,8) düzeyinde olup, postoperatif dönemde 0,85±0,73 logMAR (el hareketi-1,0) seviyesine yükseldi (p:0,001). Olguların 17sinde GİL re- pozisyonu, 11'inde GİL değişimi ve 3'ünde GİL ekstraksiyonu uygulandı. 1 gözde retina dekol- manı (%3,2), 1 gözde büllöz keratopati (%3,2), 3 gözde GİL dislokasyonu (%9,6) postoperatif erken dönemde karşılaşılan komplikasyonlardı. Sonuç: GİL dislokasyonu olan gözlerde, GİL repozisyonu veya değişimi ile görsel ve ana- tomik olarak başarılı sonuçlar elde edilmektedir. Bununla birlikte retina dekolmanı gibi ciddi komplikasyonların da olabileceği akılda tutulmalıdır.
Abstract
Aim: To evaluate the results of surgical procedures for the treatment of intraocular lens (IOL) dislocations including complications and visual outcomes. Material and Method: 31 eyes of 31 patients with IOL dislocations treated between 2008 and 2012 were retrospectively evaluated. Preoperative and postoperative best corrected visual acuity (BCVA), surgical procedures, concomitant ophthalmic pathologies, complications and follow-up time were assessed. Results: 31 eyes of 31 patients with IOL dislocations were included in this study. 23 of cases (74%) had IOL subluxation and 8 of cases (%26) had IOL luxation. Mean ages and fol- low-up times were 56.7±20.8 years (17-84 years) and 7.9±6.5 months (2-24 years), respec- tively. Preoperative mean BCVA was 1.59±1.01 logMAR (hand motion-0.8) which was signif- icantly increased to 0,85±0,73 logMAR (hand motion-1.0) (p:0,001). 17 eyes had IOL reposi- tion, 11 eyes had IOL exchange and 3 eyes had IOL extraction. Retinal detachment in 1 eye (3,2%), bullous keratopathy in 1 eye (3,2%) and IOL dislocations in 3 eyes (9,6%) were com- plications encountered during the early postoperative period. Conclusion: IOL reposition or exchange results in good anatomic and visual outcome in patients with IOL dislocations. However, it must be keep in mind that serious complications such as retinal detachment might be seen postoperatively.
