Özet

AMAÇ:Endokapsüler fakoemülsifikasyon yöntemiyle katarakt ekstraksiyonu ve göz içi lens implantasyonu uygulanan olgularda postoperatuar birinci gündeki morbiditenin ve takip kriterlerinin incelenmesi amaçlanmıştır. METOD: Aralık 1996 - Eylül 1997 tarihleri arasında kliniğimizde aynı cerrah (M.G.) tarafından endokapsüler fakoemülsifikasyon cerrahisi uygulanan ve peroperatuar komplikasyon saptanmayan 103 olgunun 122 gözü çalışma kapsamında tutuldu. Olguların 46 sı erkek, 57 si kadın olup yaşları 23 ile 82 arasında değişmekte idi (ört. 61.42 ± 11.42). Kontrol süresi 1 hafta ile 9 ay arasında (ört. 3.95 ± 2.3 ay) idi. Olgular postoperatuar birinci gün görme keskinliği, göz içi basıncı, erken postoperatuar komplikasyonlar ve eşlik eden arka segment patolojileri açısından değerlendirildiler, BULGULAR: % 70.49 olguda postoperatuar birinci gün tashihi! görme keskinliği 5/10 ve üzerinde idi. Birinci günde en sık saptanan erken postoperatuar komplikasyon kornea ödemi (% 7.38) ve göz içi basınç artışı (% 6.56), en sık eşlik eden ve görmeyi azaltan arka segment patolojisi yaşa bağlı makula dejenerasyonu (% 3.27) olmuştur. YORUM: Katarakt cerrahisinde olguların postoperatuar birinci gün değerlendirilmesinin erken postoperatuar dönem komplikasyonlarının tanınması ve erkenden tedavisi, eşlik eden görmeyi azaltıcı arka segment patolojilerinin ortaya konması ve hastaya açıklanması, cerrahın eğitimi ve kendini geliştirmesine olan katkısı gibi nedenlerle yararlı olacağı kanaatindeyiz.

Abstract

PURPOSE: In this study, the cases who had cataract extraction by endocapsular phacoemiilsification method and intraocular lens implantation have been examined, concerning the morbidity and the follow-up criteria in the first postoperative day. METHODS: 122 eyes of the 103 patients with no peroperative complications were included in this study, operated by the same surgeon (M.G.) applying the surgical techniques of endocapsular phacoemulsification. 46 of the cases were male and 57 were female, and their ages varied between 23-82. (average 61.42 ± 11.42). Monitoring period changed from 1 week to 9 months (average 3.95 ± 2.3 months). All cases were evaluated in terms of the first postoperative day visual acuity, intraocular pressure, early postoperative complications and associated posterior segment pathologies. RESULTS: The corrected visual acuity in the first postoperative day in 70.49 % of the cases was 5/10 or higher. The most frequently encountered early postoperative complication was corneal edema (7.38 %) and an increased intraocular pressure (6.56 %), while age-related macular degeneration was the most frequently posterior segment pathology diminishing the vision (3.28 %). CONCLUSIONS: It is believed that evaluation of the first postoperative day in the cataract surgery of the cases is useful for some aspects such as the diagnosis of any early postoperative complications and early treatment, diagnosis of vision diminishing posterior segment pathologies and their explanation to patients, and the training of surgeons and allowing them oppurtunities of improving his abilities.
