Özet

AMAÇ: Alkali yanık nedeniyle oluşan ciddi oküler yüzey hastalıklarında kalıcı epitel hasarını tedavi etmek ve daha düzgün bir oküler yüzey oluşturmak. YÖNTEM: Alkali yaralanma nedeniyle olgulardan 5'inde yüzey düzensizliğini, 2'sinde ise penetran keratoplasti sonrası devam eden kalıcı epitel yetersizliği tedavi etmek amacıyla 6'sı erkek, 1'i kadın olmak üzere 7 hastaya uygulandı. Hastaların ortalama yaşı 26 (18-34), ortalama izleme süresi ise 16 ay (6 31 ay) idi. Cerrahi teknik sağlam gözden alınan sağlıklı, en fazla 180°'lik kadranda olmak üzere tek serbest limbal doku greftinin hasarlı göze nakli şeklindeydi. Gerekli olgularda hasarlı göze keratektomi ve sınırlı konjonktival rezeksiyon yapıldıktan sonra konjonktival taraf 8/0, korneal taraf ise 10/0 sütürle sütüre edildi. BULGULAR: Limbal otogreft transplantasyonu sonrası vakaların hepsinde semptomlar geriledi ve sabit bir oküler yüzey elde edildi. Yedi gözün 6'sında (% 85.7) görmede artma oldu, finde (%14.3) ise görme keskinliği aynı kaldı. İnfeksiyon, limbal greft yetmezliği ya da greftin atılması gibi komplikasyonlar gözlenmedi. Bir hastada kalıcı epitel hasarı gözlendi, fakat daha sonra düzeldi. YORUM: Alkali yaralanma sonrası oluşan kalıcı epitel hasarının tedavisi ve sabit bir oküler yüzeyin keratoplasti için hazırlanması amacıyla limbal otogreft nakli diğer gözün sağlam veya az hasarlı durumlarında önerilmektedir.

Abstract

PURPOSE: To evaluate the role of limbal autograft transplantation in restoring the ocular surface in patients with ocular surface diseases and persistant epithelial defects due to alkali burns. PATIENTS and METHODS: A total of 7 patients (5 patients with alkali burns and persistent epithelial defects, 2 patients with persistant epithelial defects following penetrating keratoplasty due to alkali burn.) were treated with limbal autograft transplantation. Six (85.7%) of these patients were female. The average age of the patients were 26 years (ranging from 18 to 34). The operative technique involved transfer of free graft of limbal tissue from the uninjured eye to severly injured eye. The injured eye was prepared by limited conjunctival resection and keratectomy of fibrovascular pannus. The autograft was then transferred to recipient eye utilizing 10-0 nylon at the corneal margin and 8-0 vicryl suture at the conjunctival margin. RESULTS: After a mean of 16 months (range: 6-31) of follow-up, a stabile ocular surface was achieved in all patients associated with significant resolution of symptoms. The visual acuity increased In 6 (85.7 %)of 7 eyes, and the vision did not change in 1 patient. We did not encounter either infection, limbal graft failure, or sloughing of the grafts. Persistent epithelial defect persisted for 6 weeks in one patient, but this defect healed subsequently. CONCLUSION: Limbal autograft transplantation is an .effective surgical method for the restoration of ocular surface in patients with unilateral severe ocular surface disease due to alkali burns.
