Nekrotik ve sızdıran filtrasyon bleplerinin cerrahi olarak düzeltilmesi

Özet
Diffüz sızdıran avaskuler bleblerde ve geniş konjonktival ve skierai fleb nekrozunda tıbbi tedavi ve sütür revizyonu yetersiz kalabilir. Konjonktiva ve limbusun rekonstrüksiyonu gerekebilir. Bu durum özellikle bulber konjonktivada skarlaşma ve antimetabolit kullanımıyla daha sık görülmektedir. İki olguda açık sklerayı kapamak, bir olguda da sızdıran blebi örtmek amacıyla önce blebin yanında korneada lamellar insizyonla bölgesel yeni bir limbus meydana getirildi. Konjonktiva geriye ve yanlara doğru serbestleştirilerek, nekrozdan temizlenmiş kenarı 4-5 adet tek 8-0 ipek U-sütürleriyle korneaya gömüldü. Bu cerrahi girişim her üç hastada da başarılı oldu ve blebin daha önce mevcut olan filtrasyon fonksiyonu da devam etti. Tüm hastalarda postoperatif göziçi basıncı normal seviyedeydi ve hiçbirine antiglokomatöz medikasyon gerekmedi. Ortalama takip süresi 6.6 aydı (2-12 ay). Hiçbir hastada cerrahi komplikasyona rastlanmadı ve görmelerinde de düşme olmadı. Filtrasyon blebinin rekonstrüksiyonu ile konjonktival örtme ameliyatı, sızdıran ve nekrotik bleblerin cerrahi revizyonunda başarılı bir tekniktir.
Abstract
By diffuse leaking avascular blebs or wide conjunctiva! and scierai flap necrosis, medical treatment or suture revision may be inadequate or impossible. Reconstruction of the conjunctiva with adaptation to the limbus becomes necessary. This is especially encountered in eyes with bulbar conjunctiva! scarring and increases with the use of antimetabolites. To close the bare sciera (two cases) and the leaking bleb (one case), a new limbus at the side of the bleb was created as a lamellar incision in the cornea. The non-necrotic side of the free prepared conjunctiva was buried with 4-5, 8-0 silk U-sutures in the cornea. All three patients were successfully treated with preservation of the preexisting filtration site. The intraocular pressure control was satisfactory in all patients postoperatively. None of the patients required anti-glaucomatous medications. The average follow-up was 6.6 months (range 2-12 months). No patient had a significant decrease in visual acuity or surgical complications. The conjunctiva! covering with reconstruction of the filtering bleb is a successful technique in surgical revision of leaking or necrotic filtration blebs.
