Özet
AMAÇ: Konjonktivodakriosistorinostomi (KDSR) operasyonu fonksiyonel açıdan başarı oranı yüksek ancak postoperatuar dönemde sık komplikasyon ve buna bağlı olarak sık tüp replasmanı gereği gösteren bir girişimdir. Bu nedenle ameliyat sonrası dönemde hastaların tüpten ve yarattığı olumsuz etkilerden en kısa sürede kurtarabilmeleri amacıyla yeni arayışlar sürmektedir. Jones tüpü çevresinde bukkal mukozal greft kullanımı, sonrasında tüpün çıkarılıp pasajın epitelize kalmasıyla çözüm sağlanması yönünde girişimler son yıllarda literatürde yer almaktadır. Bu şekilde öpere edilen olguların incelenmesiyle tekniğin etkinlik ve güvenilirliği araştırılmaya çalışıldı. YÖNTEM: Ankara Numune Hastanesi 1. Göz Kliniğinde Eylül 1995 Mart 1997 tarihleri arasında 14 olguda bukkal mukozal greft'in Jones tüpü çevresine yerleştirilmesiyle gerçekleştirilen KDSR operasyonlarından alınan ilk sonuçlar değerlendirildi. BULGULAR: Olguların tamamında ortalama 13.7+5.7 aylık izlem boyunca fonksiyonel olarak başarılı sonuçlar elde edildi ve epifora yakınmaları ortadan kalktı. Postoperatuar 6. ayda tüpleri çıkarılan 11 olgudan 9'unda pasajın ilk 48 saatte kapandığı, ikisinde de açık kaldığı gözlendi. Kapanan 9 olgunun tüpleri tekrar yerleştirildiler. Replasman yapılan bu 9 olgunun 2 tanesinde operasyon sırasında mukozal greftten biyopsi alındı. Işık mikroskobik incelemelerde özellikle bukkal mukozal epitelin bazal hücrelerinde düzensizlik ve çevre bağ dokunun yuvarlak hücrelerle işgali izlendi. Bu bulgular, greftin önemli bir atılım reaksiyonuna uğradığını ve tüplerin çıkarılması sonrasında ortaya çıkan obstrüksiyonun, kalan pasajın epitelizasyonunun düzenli olmamasından kaynaklandığını kanıtlar nitelikteydiler. Tüplü dönem sırasında; 14 olgudan ikisinde latérale tüp yer değiştirmesi olurken, 1 olguda da bukkal mukozal greftin bir bölümünün ekstirpe olduğu görüldü. Bu olgulardan latérale tüp migrasyonu görülen 2 tanesinde (%14.3) cerrahi replasman gereği doğdu. Karşılaşılan komplikasyonlarda, daha önce klasik KDSR tekniği ile kliniğimizde öpere edilmiş olan 22 olguda görülen komplikasyonlarla karşılaştırıldığında, önemli oranda azalma olduğu ve cerrahi replasman gereksiniminin azaldığı saptandı. SONUÇ: Bu sonuçlar ile, bukkal mukozal greftle KDSR operasyonunun, olguların tüplü dönemlerinde komplikasyonları azalttığı ancak tüp çıkarıldıktan sonra pasaj devamlılığının sağlanması yönünde literatürde bahsedilen oranlarda başarılı olunmadığı görüldü.
Abstract
PURPOSE: Conjunctivodacryocystorhinostomy (CDCR) is a procedure with a high functional success rate, on the other hand postoperative complications are frequent and tube replacement may be required very occasionally. For this reason new inquiries are undertaken in order to save the patients from the undesired effects of the tube. Implanting Jones' tube circumscribed with buccal mucosal graft and when the passage is epithelized subsequent removal of the tube has been reported to the literature in the recent years. Efficacy and safety of this technique is discussed in this study by evaluating the cases operated using this procedure. METHODS: Results of the 14 cases who underwent CDCR with Jones' tube surrounded with buccal mucosal graft between September 1995 and March 1997 in Ankara Numune Hospital 1st Eye Clinic are evaluated. RESULTS: Functionally successful results are obtained in all cases with the cessation of epiphora complaints after a mean follow up of 13.7±5.7 months. In 11 cases tubes were removed at the 6th postoperative month, passage was obstructed in nine of them. Subsequent surgical tube replacements were performed in these patients. In two cases, biopsy specimens were taken during tube replacement and were investigated in light microscope. Especially dissarrangement of the basal epithelial cells and appearance of rejection with infiltration of surrounding connective tissues by round cells were observed. These histopathological findings were considered to be a significant graft rejection reaction and therefore trouble existed to provide the continuance of the passage. During the period with the tube, lateral tube displacement was detected in two of 14 cases and partial buccal mucosal graft extirpation was apparent in one case. Need for surgical replacement was present in only two case (14.3%) displaying lateral migration of the tube. Incidence of complications and need for surgical replacement were found to be decreased as compared with 22 patients who were previously operated in our clinic using the conventional CDCR technique.COMMENT: These findings suggest that CDCR with buccal mucosal graft is a promising new technique for the treatment of epiphora considering the lower incidence of complications at the period with the tube, however, despite the previous reports in the literature, mostly passage didn't continue when the tube was removed.
