Psödoeksfoliyasyon sendromlu olgularda ekstrakapsüler katarakt ekstraksiyonunun göz içi basıncına etkileri

Özet
Göziçi basıncı normal, psödoeksfoliyasyon sendromu ve kataraktı olan vakalarda ekstrakapsüler katarakt ekstraksiyonu ve arka kamara intraoküler lens implantasyonu (EKKE+ AK IOL) uygula¬masının göz içi basıncına etkileri incelenmiştir. Psödoeksfoliyasyon sendromu olan onaltı ve psödoeksfoliyasyonu olmayan ondokuz, normal göziçi basınçlı katarakt olgularından oluşan iki grup çalışmaya alındı. Hastaların göz içi basınçları (GİB) preoperatif son gün ve postoperatif 2 ay süresince ölçüldü.GİB her iki grupta da erken postoperatif (2 - 4 gün) dönemde anlamlı miktarda düşük bulundu.Psödoeksfoliyasyonu olan katarakt olgularında %40, kontrol grubunda ise %20 düşme gözlendi. Psödoeksfoliyasyon grubunda 2.hafta ve 2. Ayda da preoperatif GİB'a göre sı-rasıyla%13 ve % 15 düşme gözlenmiş ve istatistisel olarak anlamlı bulunmuştur. Kontrol grubu¬nun da GİB hafif düşük seyretmiş ancak erken dönemden sonraki değerlerle preoperatif GİB de¬ğerleri arasında anlamlı fark bulunamamıştır.Bu durumun muhtemel nedenleri arasında ; trabeküler debrisin katarakt ameliyatı sırasında bir miktar temizlenmesi bulunabilir. Bilindiği gibi psödoeksfoliyasyon materyalinin en azından bir kısmı lens kapsülünden kaynaklanmaktadır. Bu yüzden, ameliyatla lensin kapsülünün alınması da GİB düşüklüğünün idamesini sağlayabilir. Ay¬rıca kataraktöz lens yerine daha ince AK IOL konulması da iris lens arası mesafeyi açarak, eksfoliyasyon materyalinin saçılmasını azaltabilir ve GİB düşmesine katkı sağlayabilir. Bu çalış¬manın doğrultusunda, psödoeksfoliyasyonu olan katarakt hastalarında katarakt ameliyatı ile GİB'in düşebileceğini akılda tutmakta fayda olacağı kanısındayız.
Abstract
The affect of extracapsular cataract extraction on the intraocular pressure in cataract patients with pseudoexfoliation syndrome. This research attempts to investigate the affect of extracapsular cataract extraction and PC IOL implantation on the intraocular pressure (IOP) in cataract patients with pseudoexfoliation syndrome. This prospective , controlled study includes two groups of cataract patients. One is the group of 16 patients with pseudoexfoliation syndrome, the other is control group of 19 patients without pseudoexfoliation . All of this patient's intraocular pressures were in normal ranges. lOPs were measured preoperative last day and postoperative four times in two months period. lOPs were significantly decreased in the early postoperative period (2-4days) in each group. Mean amount of decreases are 40% and 20% in study and control groups respectively. In addition, study group's mean IOP level has significantly decreased 13 % and 15% with respect to preoperative, in postoperative second week and second month measurements respectively. IOP has continued slightly lower than preoperative measurements in two months period in the control group, and it was not significant. There may be different reasons of the IOP lowering affect of the cataract operation. Trabecular debris may be cleaned in cataract operation by means of irrigation and aspiration. Lens capsule is a partial source of the exfoliation material . Extraction of the lens capsule in the extracapsular prosedüre may lower IOP and preserve the lowering affect of the cataract operation. Artificial thin PC IOL takes place of cataracteous lens. This may result in increased distance posterior iris and lens capsule. In this way, friction between iris and capsule may be diminished. Thus, scattering amount of the exfoliative material may be reduced.ln conclusion, there must be something keeping in mind that, IOP may decreased following cataract operation especially in pseudoexfoliation syndrome.
