Özet

Amaç: Keratokonüslü kornealardaki birincil histopatolojik değişikliklerin başlangıç bölgesinin ışık ve elektron mikroskobu ile araştırılması. Yöntem: Keratokonüs nedeniyle penetran keratoplasti yapılan 18 hastadan elde edilen korneal butonlar, santral, midperifer ve periferde oluşan morfolojik değişikliklerin ayrı ayrı incelenebilmesi için 3 bölgeye ayrılarak ışık ve elektron mikroskobu ile incelendi. Bulgular: Santral korneal kesitlerde belirgin epitelyal incelmeye ve dejeneratif değişikliklere eşlik eden bazal membran ve Bowman tabakasında kırılma ve katlanmalara bağlı düzensiz bir görünüm mevcuttu. Midperiferal kesitlerde hafif epitelyal değişiklikler saptandı. Bu bölgedeki bazal epitel hücrelerinin membranları korunurken, hücreler arası bağlantı komplekslerinde yer yer ayrılmalar olduğu saptandı. Bu bulgulara eşlik eden Bowman tabakasında kalınlaşma mevcuttu. Santral ve midperiferal korneada kollajen lameller daha ince ve düzensizdi. Bu bölgelerdeki kollajen lamellerin arasındaki hücreler arası dolgu maddesinin miktarı artmıştı ve düzensiz çekirdeklerime karakterize dejenere keratositler izlenmekteydi. Stromal değişiklikler, tüm kornea boyunca derin tabakalarda daha belirgindi. Sonuç: Keratokonüsdeki erken dönem histopatolojik değişiklikler bazal epitel hücreleri arasındaki bağlantı birimlerinde ve derin stromal tabakanın, kollajeni de içeren, hücreler arası dolgu maddesinde eş zamanlı olarak ortaya çıkmaktadır. Birbirinden ayrı yerleşimli her iki bölgenin de ektodermal kökenli olması, keratokonüsün özel bir klinik tablo olmaktan çok, bir grup genetik hastalığın ortak klinik bulgusu olabileceğini düşündürmektedir

Abstract

Purpose: In this study, the site of primary histopathologic changes of cornea in keratoconus was investigated by light and transmission electron microscopy. Material: Corneal buttons were obtained from 18 keratoconus patients by penetrating keratoplasty, and were dissected into 3 parts to examine the morphology in central, midperipheral and peripheral cornea separately. Results: The central corneal sections showed marked epithelial thinning with degenerative changes in the basal cells, disintegration of intercellular junctional complexes and an irregular appearance of basement membrane and Bowman's layer due to breaks and folds. Midperipheral corneal sections showed mild epithelial changes. Junctional complexes of the basal cells of midperipheral cornea were partly dissolved while the cell membranes remained intact. An accompanying thickening in Bowman's layer was also noted. Collagen lamellae were more irregular and thin in central and midperipheral cornea, and between them there was an increased amount of extracellular matrix with degenerated keratocytes. Stromal changes were more significant in deeper stromal layers throughout the cornea. Descement's membrane and endothelium were normal. Conclusion: The earliest histopathologic changes in keratoconus appear to start from the junctional complexes of basal epithelial cells and the extracellular components of deep stroma, including collagen fibrils, simultaneously. Since both of these separately located sites differentiate from ectoderm, we conclude that keratoconus may be a common clinical sign for a group of genetic disorders, rather than being a specific corneal disease.
