Behçet hastalığında katarakt cerrahisi sonuçlarımız ve postoperatif görme keskinliği düşüklüğü nedenleri

Özet
AMAÇ: Üveitli olgularda katarakt ekstraksiyonu halen tartışmalı bir konudur. Bu çalışmanın amacı, katarakt ekstraksiyonu yapılan Behçet hastalığı olgularının postoperatif cerrahi ve görme keskinliği sonuçlarının değerlendirilmesidir. YÖNTEM: Katarakt cerrahisi uygulanan 12 hastanın 13 gözü çalışma kapsamına alındı. 8 olguya planlı ekstrakapsüler katarakt ekstraksiyonu (PEKKE), 3 olguya pars plana vitrektomi-lensektomi (PPV-lensektomi) ve 2 olguya da PEKKE+Göz içi lens implantasyon cerrahisi yapıldı. Olgu grubunun yaşları ortalama 39.33±3.44 yıl (22-60) idi. Postoperatif takip periodu 26.3±4.58 ay idi. BULGULAR: Postoperatif görme artışı 6 (%46) olguda saptandı. Çalışma grubunda Snellen eşeline göre ortalama 2.6 sıra artış vardı. 2 (%39) olguda görmede azalma bulunurken, 5 (%39) olguda görme değişikliği yoktu. Üveit ilk yılda 5 (%38) vakada rekürrens gösterdi. Postoperatif komplikasyonlar arasında en sık posterior sineşi (%76) görüldü. Bunu optik atrofi (%46) ve glokom (%23) takip etti. Retinada premaküler membran ve makulada lameller delik, birer olguda (%7.6) saptandı. SONUÇ: İnflamasyon cerrahi öncesi tam olarak kontrol edilse bile Behçet hastalığına bağlı komplike katarakt olgularında prognoz, diğer üveit gruplarına göre daha kötü seyretmektedir. Preoperatif gelişen optik sinir-retina patolojileri, glokom ve inflamasyon; kalıcı harabiyete neden olmakta ve postoperatif görme keskinliğini düşürmektedirler.
Abstract
PURPOSE: Cataract extraction in uveitis patients has been a controversial issue. The purpose of this study is to evaluate the postoperative surgical findings and visual outcome of cataract extraction in patients with Behçet's disease. METHODS: The results of 13 cases of cataract extraction in 12 patients with Behçet's disease were analyzed. Eight cases had planned extracapsular cataract extraction (PECCE), 3 cases had PPV-lensectomy, 2 cases had PECCE and intraocular lens (IOL) implantation. The mean age of the patients was 39.33±3.44 years (22-60 years). The mean postoperative follow-up period was 26.3 ± 4.58 months. RESULTS: Visual acuity improved postoperatively in 6 eyes (46%). The mean of this improvement was 2.6 Snellen lines. Visual acuity decreased in 2 eyes (15%), and there was no change in visual acuity in 5 eyes (39%). Uveitis recurred in 5 eyes (38%) within the first postoperative year. The most common postoperative complication was posterior synechia formation (76%) followed by optic atrophy (46%) and glaucoma (23%). Premacular membrane in the retina and lamellar hole in the macula were present in one case, each (7.6%). CONCLUSION: Despite adequate control of the inflammation preoperatively, surgical outcome in patients with Behçet's disease is worse than in patients with other forms of uveitis. Pre-existing pathologies of the optic nerve and the retina, glaucoma and uncontrolled inflammation lead to permanent ocular damage and are the most important factors to decrease the postoperative visual acuity.
