Özet
AMAÇ: Topikal anestezi ile trabekülektomi ameliyatının yeterliliğini ve güvenilirliğinin araştırılması. GEREÇ VE YÖNTEM: Trabekülektomi uygulanacak ileri evre glokomlu 22 olguya (l. Grup) sponca emdirilmiş %0.5 Bupivacain HCI (Marcain) üst fornikse yerleştirilerek 15 dakika süre ile beklendi ve ameliyat sırasında hasta uyumu, cerrahinin uygulanabilirliği ağrı miktarı ve topikal anesteziye bağlı komplikasyonlar yönünden değerlendirildi. Ameliyat aşamalarında sorulduğu zaman ağrının hastalar tarafından yok ise 0, hafif ise 1, orta dereceli ise 2 ve şiddetli ise 3 şeklinde derecelendirmeleri istendi. Yirmi olguluk retrobulber anestezi (II. Grup) ile uygulanan trabekülektomi grubu ile ağrı ve komplikasyon yönünden karşılaştırıldı. BULGULAR: Topikal anestezi uygulanan olguların hepsinde sponc alındıktan sonra konjonktivada hiperemi izlendi. Üst rektus sütürünün konulması, forniks tabanlı konjonktivanın açımı ve koterizasyon, flep kaldırılması, ve trabekülektomi ostiumunun açılması esnasında ağrı yönünden gruplar arasında istatistiksel olarak anlamlı bir farklılık yoktu, iris prolapsusu sırasında topikal anestezi grubunda ağrı daha fazla idi ancak istatistiksel anlamlı değildi. İridektomi aşamasında ağrı topikal anestezi grubunda istatistiksel olarak anlamlı bir şekilde fazla idi (Fischer exact test t = 0.0006) . Konjonktiva kapatılması ve postop. ağrı yönünden yine gruplar arasında fark saptanmadı. Ameliyat sırasında topikal anesteziye bağlı bir komplikasyon ile karşılaşılmadı. Retrobulber anestezi grubunda 20 olgunun 8'inde retrobulber anesteziyi takiben geçici görme kaybı tanımlandı. Retrobulber anestezi uygulanacağı için oral antikoagulanı kesilen bir olguda geçici atak gelişti. Bunun dışında oküler bir komplikasyon ile karşılaşılmadı. SONUÇ: Ön segment cerrahisinde topikal anestezinin yeterli ve güvenli bir metod olduğu kanısına varıldı.
Abstract
PURPOSE: To search for the efficiency and reliabllity of trabeculectomy operation with topical anesthesia. MATERIALS AND METHODS: 0.5% Bupivacain HCI (Marcain) was absorbed by sponge and placed into the upper fornices for 15 minutes in 22 patients with high stage glaucoma those to be applied trabeculectomy (Group l). During the operation; patient cooperation, applicability of surgery, the amount of pain and complications due to topical anesthesia were evaluated . The patients were asked to graduate the pain if there is no pain 0, mild 1, moderate 2 and severe 3 and compared with 20 patients to whom trabeculectomy was perfored with retrobulbar anesthesia(Group II) for the pain sensation and complication. FINDINGS :In all patients with topical anesthesia, hiperemia is seen in the conjunctivas of all patients after the sponge is taken away. There is no difference for pain sensation between two groups, during the suturation of upper rectus, fornix based conjunctival opening, cauterization and removing flep. However, greater pain is felt at the stage of iris prolapsus through the ostium of trabeculectomy, in topical anesthesia grup but this is not statistically significant. At the stage of iridectomy greater pain is felt in the topical anesthesia group . This difference was statistically significant (t=0.0006). There was no difference between two groups during the conjunctiva suturation and postoperative pain. During the operation, no complication related to topical anesthesia was seen. In retrobulbar anesthesia group transient visual loss were seen in 8 patients. There is no other complication because of retrobulbar anesthesia except for one patient who had stroke because of discontinuation her oral anticoagulant therapy RESULTS: It is believed that topical anesthesia is a sufficient and confident method during anterior segment surgery.
