Katarakt hastalarında postoperatif vizyon tahmininde Heine retinometrenin yeri

Özet

AMAÇ: Bu çalışmada katarakt cerrahisi uygulanan olguların cerrahi sonrası düzeltilmiş en iyi görme keskinliklerini, operasyon öncesi tahmin etmek amacıyla Heine lambda 100 el retinometresinin güvenilirliği araştırıldı. GEREÇ VE YÖNTEM: Yüzellisekiz olgunun 160 gözünde preoperatif Heine lambda 100 retinometre kullanılarak tahmin edilen görme keskinliği değerleri, postoperatif düzeltilmiş en iyi görme keskinliği değerleri ile karşılaştırıldı. BULGULAR: Snellen eşelinde 2 sıra yanılma payı ile doğru tahmin oranı %53.2, yalancı negatif (3 sıra ve altı yanılma) oranı %45.2, yalancı pozitif (3 sıra ve üzeri yanılma) oranı %1.6 olarak saptandı. Preoperatif düzeltilmiş en iyi görme keskinliği ışık hissi (P+P+) ve el hareketleri düzeyindeki olgularda (Grup I) doğru tahmin oranı %17.9, yalancı negatif oranı %82.1, yalancı pozitif oranı %0.0, preoperatif düzeltilmiş en iyi görme keskinliği 0.1 ve üstündeki olgularda (Grup II) doğru tahmin oranı %81.0, yalancı negatif oranı %14.3, yalancı pozitif oranı %4.8 olarak bulundu. İki grup arasındaki doğru tahmin edilebilirlik farkı istatistiksel açıdan ileri derecede anlamlıydı (p=0.000). SONUÇ: Heine lambda 100 retinometre taşınabilir ve diğer örneklerine oranla maliyeti daha ucuzdur. Kolay uygulanabilme avantajına sahip olup, katarakt operasyonu sonrasındaki görsel sonuç tayininde güvenilir bir yöntemdir. Matür olmayan karakterdeki katarakt olgularında %80'in üzerinde doğru tahmin oranına sahiptir ancak, uygulanması sırasında yalancı pozitif sonuç yaratabilecek olgularda çok dikkatli olmak gereklidir.

Abstract

PURPOSE: In this study, Heine lambda 100 handheld retinometer's reliability in the prediction of the best corrected postoperative visual acuity in the patients to whom cataract surgery was applied was investigated. MATERIAL AND METHODS: The predicted preoperative visual acuity with Heine lambda 100 handheld retinometer in 160 eyes of 158 patients were compared with the postoperative best corrected visual acuity. RESULTS: It was determined that at Snellen Chart the correct prediction ratio with the accepted error ratio was 53.2%, false (-) (3 lines and less error) ratio was 45.2%, false (+) (3 lines and more error) ratio was 1.6%. The correct prediction ratio of postoperative visual acuity of the patients with visual acuity of light perception or hand movements was 17.9%, false (-) ratio was 82.1%, false (+) ratio was 0.0%. It was found that the correct prediction ratio of the patients whose best improved preoperative visual acuity was 0.1 and more (Group II) was 81.0%, false (-) ratio was 14.3%, false (+) ratio was 4.8%. The difference in correct prediction between two groups was statistically significant (p=0.000). CONCLUSIONS: It is estimated that, besides Heine lambda 100 retinometer's tangibility, cheapness when compared with its other examples, easily applicability, this instrument is reliable in the assignment of visual prognosis after cataract surgery. It has a 80% correct prediction rate in postoperative visual acuity of mild to moderate cataract patients but one must be careful of the false (+) results that can occur in some cases.
