Emetrop ve miyop gözlerde aksiyel uzunluk ve kornea refraktif parametrelerinin karşılaştırılması

Özet
AMAÇ: Refraktif durumu belirleyen parametreler; kornea kurvatür yarıçapı, aksiyel uzunluk, ön kamara derinliği ve lens kırma gücüdür. Bu çalışmada emetrop ve miyop gözlerde aksiyel uzunluk ve kornea refraktif parametrelerinin (santral kornea kurvatür yarıçapı, santral kornea kırma gücü, santral kornea kalınlığı) karşılaştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Kırma kusuru olmayan (emetrop) 30 olgu (30 göz) ve kırma kusuru -7 D ile -10 D arasında değişen 30 miyop olgu (30 göz) çalışma kapsamına alınmıştır. Aksiyel uzunluk A-scan ultrasonografi ile, santral kornea kalınlığı ultrasonik pakimetre ile, santral kornea kırma gücü komputerize videokeratografi ile, santral kornea kurvatür yarıçapı otokeratorefraktometre ile ölçülmüştür. BULGULAR: Santral horizontal ve santral vertikal kornea kurvatür yarıçapı emetrop gözlerde miyop gözlere göre anlamlı derecede yüksek bulunmuştur (p=0.05 ve p=0.0012). Aksiyel uzunluk, miyop gözlerde emetrop gözlere göre anlamlı derecede yüksek bulunmuştur (p<0.001). Santral kornea kırma gücü miyop gözlerde emetrop gözlere göre daha yüksek bulunmakla birlikte, gruplar arasındaki bu fark istatistiksel olarak anlamlı saptanmamıştır (p=0.065). Santral kornea kalınlığı açısından miyop ve emetrop gözler arasında önemli bir fark olmadığı bulunmuştur (p=0.623). SONUÇ: Bizim çalışmamızda aksiyel uzunluk ve santral kornea kurvatür yarıçapı refraktif durumu belirleyen önemli parametreler olarak bulunmuştur. Miyop gözlerde santral kornea kurvatür dikliği ve aksiyel uzunluk emetrop gözlere göre daha yüksek bulunmuştur. Ön kamara derinliği ve lens kırma gücü gibi refraktif durumu etkileyebilecek diğer parametrelerin ileri teknikler kullanılarak geniş serilerde değerlendirilmesini önermekteyiz.
Abstract
PURPOSE: The refractive state of the eye is dependent on parameters like; corneal curvature radius, axial length, anterior chamber depth and lenticular power of the globe. The present study was conducted to compare axial length and corneal refractive parameters (central corneal curvature radius, central corneal power, central corneal thickness) in emmetropic and myopic eyes. MATERIAL AND METHODS: Thirty subjects (30 eyes) with no refractive error (emmetrope) and 30 subjects (30 eyes) having from 7 D to 10 D of myopia were selected for the study. Axial length was measured using A-scan ultrasonography, central corneal thickness was measured by ultrasound pachymetry, central corneal power was measured by computer asisted videokeratography, central corneal curvature radius was measured in each patient with an autokeratorefractometer. RESULTS: Central horizontal and central vertical corneal curvature radii for the emmetropic eyes were significantly greater than that for the myopic eyes (p=0.05 and p=0.0012). Axial length for the myopic eyes was significantly greater than that for the emmetropic eyes (p<0.001). Central corneal power of the myopic eyes was greater than that of the emmetropic eyes but the correlation between two groups was not statistically significant (p=0.065). No significant differences in central corneal thickness were observed between the myopic and emmetropic eyes (p=0.623). CONCLUSIONS: Our study has shown that axial length and central corneal curvature radius are two important parameters effecting the refractive state of the eye. Myopic eyes had steeper central corneal curvature and greater axial length than emmetropic eyes. We recommend that other parameters like anterior chamber depth and lenticular power that can influence the refractive state of the eye be studied with advanced techniques in large series.
