Özet
AMAÇ: Yakın okuma güçlüğü ve göz yorgunluğu şikayetleri ile başvuran genç erişkinlerde akomodasyon bozuklukları, heteroforya ve konverjans yetmezliği gibi astenopiye yol açabilecek nedenlerin araştırılması. GEREÇ VE YÖNTEM: Astenopi şikayeti ile başvuran üniversite öğrencilerinin oluşturduğu birinci grup ile kızarıklık, sulanma şikayetleriyle veya ehliyet almak için başvuran olgulardan oluşan ikinci grupta, tam göz muayenesi yapılmasının ardından dinamik retinoskopi ile akomodasyon genlikleri, prizma ile akomodatif konverjans genlikleri ve cetvelle yakın konverjans noktası ölçüldü. İki gruptan elde edilen sonuçlar istatistiksel olarak karşılaştırıldı. BULGULAR: Birinci gruptaki (n=48) olguların görme keskinliği (sağ:0.98±0.07, sol: 0.98±0.07), statik retinoskopi (sağ:0.22±0.16D, sol:0.20±0.15D) ve dinamik retinoskopi (sağ:0.38±0.10D, sol:0.37±0.12D) değerleri ile ikinci gruptaki olgulara (n=42) ait görme keskinliği (sağ:0.99±0.02, sol: 0.99±0.03), statik retinoskopi (sağ:0.15±0.18D, sol:0.14±0.17D) ve dinamik retinoskopi (sağ:0.28±0.13D, sol:0.29±0.12D) değerleri arasında istatistiksel farklılık yoktu (student-t testi). Birinci grupta, bir olguda 10 PD ekzoforya mevcuttu. Yakın konverjans noktası ve akomodatif konverjans genlikleri gruplar arasında farklılık göstermemekteydi. SONUÇ: Çalışmamızda ekzoforya ve konverjans yetmezliği gruplar arasında istatistiksel fark olmaksızın tesbit edilmiş olmasına karşın, akomodasyon bozukluğuna hiçbir olguda rastlanmamıştır. Astenopi yakınması ile gelen olgularda, eşlik eden sistemik hastalık veya ilaç kullanımı öyküsü olmadığında, etken olabilecek düzeltilmemiş kırma kusuru, heteroforya, konverjans yetmezliği gibi göze bağlı patolojilerin dışlanması sonrası eşlik eden fizyolojik mekanizmalar hakkında hastaya veya aileye bilgi verilmesi tedavinin temelini oluşturur.
Abstract
PURPOSE: To investigate the pathologic causes such as accomodation insufficiency, heterophoria and convergence insufficiency in young adult population who have the complaints of ocular fatigue and difficulty in near vision. MATERİALS AND METHODS: Group 1 consisted of university students that were applying with complaints of asthenopia and the second group consisted of cases that were seen with the complaints of redness, epiphora and for driving license examination. After complete ophthalmological examination, accomodation amplitudes were measured with dynamic retinoscopy, accomodative convergence amplitudes with prisms and near convergence points with ruler. The results that were obtained from two groups were compared statistically. RESULTS: The visual acuity (right:0.98±0.07, left: 0.98±0.07), static retinoscopy (right:0.22±0.16D, left:0.20±0.15D) and dynamic retinoscopy (right:0.38±0.10D, left:0.37±0.12D) values in the first group (n=48) showed no statistically significant difference from the second group's (n=42) visual acuity (right:0.99±0.02, left:0.99±0.03), static (right:0.15±0.18D, left:0.14±0.17D) and dynamic retinoscopy (right:0.28±0.13D, left:0.29±0.12D) values (student-t test). Exophoria of 10 PD was diagnosed in one case in the first group. There was no significant difference between the groups in terms of near convergence point and accomodative convergence amplitudes. CONCLUSION: Even though exophoria and convergence insufficiency were detected without statistical significance, there was no accomodative pathology as a result of our study. After elimination of possible pathological etiological factors such as uncorrected refractive errors, heterophoria, convergence insufficiency and systemic diseases or medication, information about associating physiological mechanisms that is given to the patients or the family is the mainstay of the treatment.
