Yüksek hipermetroplarda hipermetropi, anizometropi ve ambliyopinin binoküler fonksiyonlar üzerine etkisi

Özet

AMAÇ: Ortoforik yüksek hipermetroplarda binoküler fonksiyonların değerlendirilmesi GEREÇ VE YÖNTEM: 3 diyoptrinin üzerinde hipermetropi tespit edilen ve şaşılık hikayesi olmayan 56 hipermetropik olgu araştırmaya alındı. Olgularda ilk gözlük takma yaşı belirlendi. Olgular hipermetropi derecesine (3-5 diyoptri arası veya >5 diyoptri), anizometropi (1.5 diyoptri fark) ve ambliyopi (Snellen eşeline göre 2 sıra fark) varlığına göre gruplanarak füzyon (periferal ve santral) ve stereopsis (iyi, £100 arc / sn; kaba, > 100 arc / sn) açısından karşılaştırıldı. BULGULAR: İlk gözlük takma yaşı ortalama 12.54±10.09 yıl olarak bulundu. Anizometropi ve ambliyopiye sahip olmayan ve hipermetropisi 3-5 diyoptri arasında değişen 8 olguluk 1. grupta iyi stereopsis gelişim oranı %75, santral füzyon gelişim oranı %100 iken; hipermetropisi 5 diyoptrinin üzerinde olan 12 olguluk 2. grupta bu oranlar %16.7 ve %83.3 olarak bulundu. Hipermetropideki artışın stereopsis gelişimini anlamlı derecede etkilediği (p 0.05) saptandı. Sekiz olguda anizometropi saptandı. Anizometropik grupta %12.5 iyi stereopsis ve %87.5 santral füzyon gelişimi izlenirken; anizometropik olmayan grupta (n=48) bu oranlar %29 ve %92 idi. Anizometropik olmayan grup binokülarite açısından daha iyi bulunsa da anizometropinin stereopsis (p>0.05) ve füzyon (p>0.05) gelişimi açısından istatistiksel önemi olmadığı saptandı. Otuzaltı olguda unilateral veya bilateral ambliyopi saptandı. Ambliyopik grupta %5.5 iyi stereopsis ve %88 santral füzyon saptand ı. Ambliyopik olmayan grupta (n=20) ise bu oranlar sırasıyla, %60 ve %95 idi. Ambliyopinin iyi stereopsis gelişimini etkileyen önemli bir faktör olduğu saptanırken (p 0.05) önemli bulunmadı. SONUÇ: Yüksek hipermetropik kusur ve anizometropinin görsel gelişim periyodunun başında düzeltilmemesi yüksek oranda unilateral veya bilateral ambliyopi gelişimine neden olması yanında, kayma olmasa bile binoküler gelişmeyi ve özellikle stereopsis gelişimini olumsuz yönde etkilemektedir.

Abstract

PURPOSE: To evaluate binocular visual functions in ortophoric high hypermetropic patients. MATERIAL AND METHODS: Fiftysix ortophoric patients with hypermetropia greater than or equal to 3 D are studied. The age at first optical correction is asked. Patients are classified by degree of hypermetropia (between 3-5 D or >5 D), presence of anisometropia (difference of 1.5 dioptres or more) and amblyopia (difference of two lines on Snellen chart). Sensory status is assessed by measuring stereoacuity and Worth four dot fusion in different groups. RESULTS: The mean age at first optical correction was 12.54±10.09 years. In the first group (n=8) without anisometropia and amblyopia, degree of hypermetropia was between 3-5 D, and 75% of patients had fine stereopsis and all of the patients had central fusion; in the second group whose degree of hypermetropia was greater than 5 D (n=12), 16.7% of patients had fine stereopsis and 83.3% had central fusion. It was found that increase in the degree of hypermetropia affected the development of stereopsis (p<0.05), but had no effect on fusional development (p>0.05). Anisometropia was detected in 8 patients. In anisometropic group, 12.5% of patients had fine stereopsis and 87.5% of them had central fusion. In patients without anisometropia 29% of them had fine stereopsis and 92% of them had central fusion. Though anisometropia may cause abnormalities in binocular visual functions, it was not found to be statistically significant (p>0.05). Amblyopia was detected in 36 patients. While 5.5% of patients had fine stereo psis and 88% of patients had central fusion in the amblyopic group, 60% of patients had fine stereopsis and 95% of them had central fusion in the nonamblyopic group. It was found that amblyopia is a statistically significant factor on the development of stereopsis (p<0.00001), but not found to be significantly important on fusional development (p>0.05). CONCLUSION: Uncorrected high degree of hypermetropia and anisometropia at the initial stage of visual development causes unilateral or bilateral amblyopia and disturbs binocular visual functions, especially stereopsis, even in the absence of strabismus.
