Özet
Genel anestezi altında sezeryan ile doğum yapan 41 yaşındaki hasta, doğumdan sonra bir hafta içinde her iki gözde giderek artan orta şiddette ağrı ile birlikte görme kaybı nedeniyle kliniğimize sevkedildi. Kliniğimize başvurduğunda bir aylık görme kaybı öyküsü bulunan hastada, bilateral tam açı kapanması ve 50 mm Hg'nın üzerinde göziçi basınçları (GİB) ile birlikte ön segmentlerin sakin olduğu ve iki taraflı optik atrofi geliştiği gözlendi. Tıbbi tedaviye başlanarak bilateral lazer iridotomi yapılan hasta bu tedavilerden fayda görmedi ve sürdürülmekte olan bir çalışma dahilinde, bir gün arayla sağ göze trabekülektomi, sol göze viskokanalostomi ameliyatları yapıldı. Postoperatif 12. ayda her iki gözde GİB'ları ilaçsız olarak kontrol altındaydı. Literatürde doğum eylemi ve genel anestezi ile ortaya çıkan akut açı kapanması glokomu olguları bildirilmiştir. Sunduğumuz vaka ise genel anestezi altında sezeryan sonrası bildirilmiş ilk açı kapanması glokomu vakası olma özelliği taşımakta; yatkınlığı ve aile öyküsü bulunan olgularda bu açıdan dikkatli olunmasının önemini vurgulamaktadır.
Abstract
A 41 year-old woman who underwent caesarean section under general anesthesia was referred with bilateral moderate ocular pain and visual deterioration that gradually worsened during the first week after labor. The patient had a one-month-history of visual impairment, her intraocular pressures (IOP) were higher than 50 mmHg in both eyes -with bilateral total angle-closure, anterior segments were quiet and optic atrophies were established bilaterally. Medical treatment was initiated and bilateral laser iridotomies were performed. They were not successful to resolve the condition therefore trabeculectomy was performed to the right eye and viscocanalostomy was performed to the left eye, and intraocular pressures were under control without medicine at the 12th postoperative month. Precipitation of acute angle-closure glaucoma with uncomplicated labor and general anesthesia has been reported. As far as we know, this is the first case of precipitation of angle-closure glaucoma following caesarean section under general anesthesia reported to the literature and the significance of the condition in predisposed cases and patients with a family history is emphasized
