Skleral Fiksasyon ile Arka Kamara Lens İmplantasyonunda Haptik Konumlarının Tilt ve Desantralizasyon Üzerine Etkisi

Özet

Amaç: Skleral fiksasyon ile arka kamara lens implantasyonu (SF-AKL) uygulanan gözlerde oluşan postoperatif tilti ve desantralizasyonu etkileyen faktörlerin belirlenmesi ve göziçi lens (GİL) malpozisyonunun postoperatif refraksiyon kusurları ve astigmatizma üzerineolan etkilerinin incelenmesi. Gereç ve Yöntem: SF-AKL yapılan 21 olgunun 24 gözü (SF grubu) ve fakoemülsifikasyoncerrahisi uygulanarak kapsüler kese içine GİL yerleştirilen 24 olgunun 24 gözü (kontrol grubu) postoperatif tilt, desantralizasyon, astigmatizma ve sferik eşdeğer parametreleri yönünden karşılaştırıldı. SF grubundaki gözler ultrason biyomikroskopisi ile incelenerek, her ikihaptiği de siliyer sulkusta yerleşen gözler en az bir haptiği sulkus dışında olan gözlerle aynıparametreler yönünden alt grup analizi ile karşılaştırıldı.Bulgular: SF grubunda saptanan tilt ve desantralizasyon miktarları kontrol grubuna göre anlamlı olarak yüksek bulundu (her iki parametre için p<0,001). GİL'ine bağlı rezidüel astigmatizma (rA) SF grubunda kontrol grubuna göre anlamlı olarak daha yüksekti (p=0,014).Tüm gözler birlikte değerlendirildiğinde, rA değeri ile tilt ve desantralizasyon miktarları arasında anlamlı bir ilişki olduğu saptandı (sırasıyla, p<0,001 ve p=0,044). Çoklu regresyonanalizi ile sadece tilt miktarındaki artışın rA miktarını istatistiksel anlamlı olarak artırmayadevam ettiği saptandı (p<0,001). SF grubunda, GİL'inin her iki haptiğinin de siliyer sulkusta yerleştiği gözlerde tilt ve rA miktarı en az bir haptiği sulkus dışında yerleşenlere göre anlamlı olarak daha az bulundu (sırasıyla, p=0,005 ve p<0,001).Sonuç: Skleral fiksasyon cerrahisi sonrasında GİL malpozisyonu ve buna bağlı astigmatizma fakoemülsifikasyon cerrahisine göre daha yüksek oranda oluşmaktadır. Bu sorun, GİLhaptiklerinin her ikisinin de siliyer sulkus bölgesine yerleştirilmesi ile azaltılabilmekte ancaktamamen ortadan kaldırılamamaktadır. Kapsül desteği olmayan gözlerde SF ile implante edilen GİL'lerinin stabilitesi için mevcut cerrahi yöntem ve malzemelerin daha fazla geliştirilmesine ihtiyaç vardır

Abstract

Aim:To determine the factors that influence postoperative tilt and decentralization in eyesthat underwent posterior chamber lens implantation with scleral fixation (SF), and to inves-tigate the effects of intraocular lens (IOL) position on postoperative refractive errors andastigmatism. Material and Method:Twenty-four eyes of 21 patients that underwent SF (SF group), and24 eyes of 24 patients in which IOLs were implanted in capsular bags after phacoemulsifica-tion (control group) were compared in terms of postoperative tilt, decentralization, astigma tism, and spherical refractive errors. Eyes in the SF group were examined with ultrasoundbiomicroscopy and the eyes in which both IOL haptics were located in the ciliary sulcus werecompared with eyes in which at least one haptic was out of the ciliary sulcus regarding thesame parameters as two separate subgroups. Results:Tilt and decentralization were significantly to a higher extent in the SF group(p<0.001 for both parameters). Residual astigmatism (rA) related to IOL was also signifi-cantly higher in the SF group (p=0.014). When all eyes were taken into consideration, signif-icant correlations existed between rA and the measure of tilt and decentralization (p<0.001and p=0.044, respectively). Regression analysis in a multivariate model revealed that only theincrease in the extent of IOL tilt was significantly related with the increase in rA (p<0.001).In the SF group, grades of tilt and rA were significantly lower in eyes in which both hapticswere located in the ciliary sulcus compared to the eyes in which at least one haptic was outof the ciliary sulcus (p=0.005 and p<0.001, respectively). Conclusion:The rate of IOL position failure and related astigmatism is higher followingscleral fixation compared to intracapsular implantation with phacoemulsification. This draw-back might be minimized when both IOL haptics are positioned in the ciliary sulcus but maynot be eliminated completely. Current surgical techniques and materials need to be improvedto increase the stability of IOLs implanted with SF in eyes without capsular support
