Özet
Bu çalışmada, 1963-1994 yıllan arasında arasında Ankara Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Oküler Onkoloji Bölümünde izlenen 636 retinoblastom (RB) olgusunun tedavi özellikleri ve prognozu incelenmiştir. 441 olgu tek taraflı, 195 olgu ise iki taraflıdır. Tek taraflı RB olgulannda en fazla tercih edilen başlangıç tedavi enükleasyondur (412 olgu). Bu olgular arasında optik sinir tutulumu görülen ve/veya en-ükleasyondan sonra orbitada yineleyen olgularda idame tedavi olarak ekzanterasyon (48 olgu), radyoterapi (154 olgu) ve kemoterapi (108 olgu) uygulanmıştır. 17 olguda ilk başvuru sırasında kütlesel proptosis, bul-bus yıkımı ve körlük gibi nedenlerle ekzanterasyon ameliyatı yapıl¬mıştır. İki olguda ise cerrahi yöntem uygulanmadan dışsal ışın tedavisi yapılmıştır. On olguda tedavisiz kendiliğinden gerileme saptanmıştır. İki taraflı RB'da en çok uygulanan başlangıç tedavi, bir göze enüklea-syon ve diğer göze radyoterapidir (132 olgu). İzlemde orbitada yineleme ve/veya sistemik yayıhm durumuna göre bu olgulardan 9'una ekzante¬rasyon. 50'slne kemoterapi yapılmıştır. Bir olguda iki taraflı kendiliğin¬den gerileme gözlenmiştir. Altı olguda iki taraflı ışın tedavisi yapıl¬mıştır. Geri kalan 56 olgunun bir gözlerine enükleasyon girişimi uygu¬lanmıştır. Diğer gözlere uygulanan tedavi yöntemleri Co plak (18 olgu: 10 olguda primer, 8 olguda sekonder tedavi olarak), krio (14 olgu), en¬ükleasyon (11 olgu), fotokoagtilasyon (6 olgu) ve ekzanterasyon (1 olgu) dur. 14 olguda diğer göze herhangi bir tedavi uygulanmamıştır. En az 18 ay ve üstünde izlenen olgularda 25 tedavi komplikasyonu saptan¬mıştır. Bunların 18'i ışın kataraktı olup ameliyat edilmiş ve 6 olguya aynı seansta göz içi lensi uygulanmıştır. İki taraflı RB'u olan 2 olguda dışsal ışın tedavisi sonucu orbitada osteosarkom gelişmiştir. Bu olgular osteosarkom görüldüğü zaman 14 ve 15 yaşlarında idiler. Üç olguda ışına bağb fitizis bulbi, iki olguda da ışın keratiti saptanmıştır. Ortala¬ma 5 yıllık izlem süresi sonunda 636 RB olgusundan 113'ü (%17.8) eksi-tus olmuştur.
Abstract
The treatment modalities and prognosis of 636 retino¬blastoma (RB) cases diagnosed at the Ankara Universi¬ty Eye Clinic Ocular oncology Department were eva¬luated of the 636 cases. 441 were unilateral RBs (412 cases). Follow-up treatment included exenteration (48 cases), radiotherapy (154 cases) and chemotherapy (108 cases) for patients with optic nerve invasion and/or orbital recurrence following enucleation. Se¬venteen cases displayed massive proptosis, ocular da¬mage and blindness at initial presentation and under¬went exenteration as the initial treatment. Two cases were subjected to external irradiation treatment wi¬thout invasive surgical procedures. Ten cases regres¬sed spontaneously without treatment. For bilateral cases, the most frequent treatment used was enuclea¬tion for one eye and' radiotherapy for the other (132 cases). Adjuvant treatment included exenteration (9 cases) and chemotherapy (50 cases) depending on or- bital recurrence and/or systemic metastasis. Sponta¬neous bilateral regression was noted in one case.Six cases underwent bilateral external radiotherapy. One eye of the remaining 56 cases underwent enucleation. The treatment for the other eyes included Co plaque therapy in 18 cases (10 as primary treatment and 8 as secondary treatment after other failed therapy), criotherapy in 14 cases, enucleation in 11 cases, photocoagulation in 6 cases and exenteration in one case. No treatment was un¬dertaken in the other eyes of 14 cases. Treatment com¬plications were detected in 25 cases followed for at least 18 months. Eighteen cases had radiation cataracts and six of these 18 patients underwent intraocular lens imp¬lantation. Post-radiation malignancy (osteosarcoma) was noted in two cases aged 14 and 15 years. Phthisis bulbi was noted in three cases and radiation keratitis in two cases. 113 cases (17.8%) out of 636 were deceased at a mean follow-up of 5 years.
