Özet

Ciddi oküler ve santral sinir sistemi hasarına yol açan akut metil alkol intoksikasyonunda yeterli öykü alınması genellikle mümkün değildir. Bu nedenle klinik, laboratuvar ve radyolojik bulguların bilinmesi, etkin antitoksik tedavinin uygulanmasına, dolayısıyla yaşamın ve görmenin kurtarılmasına yol açabile¬ceğinden önemlidir. Önemli bir belirti olan görme keskinliğinin azalması, bazen tek bulgu olabilir. Bu çalışmada; dört ay içinde üç ayrı klinikte görülen ve kolonyanın içki olarak tüketilmesi sonucunda ortaya çıkan 9 metil alkol intoksikasyonu olgusunda saptanan oküler ve radyolojik bulgular sunulmuştur. Olguların 7'si körlük ile 2'si ise eksitus ile sonuçlanmıştır. Metanol in¬toksikasyonu bulguları olan peripapiller sinir lifi ödemi 4 olgu¬da, putamen nekrozu 2 olguda gösterilmiştir. Elektrofîzyolojik testlerin uygulanabildiği olgularda elektroretinografide a ve b dalgalarının amplitüd ve latanslarının normal ve flash vizuel evoked potansiyel dalgalarının düzleşmiş olduğu saptanmıştır. Erken tanıda göz hekiminin rolü vurgulanarak antitoksik tedavi yöntemleri tartışılmıştır.

Abstract

It is usually impossible to obtain an adequate history in methyl alcohol intoxication which causes severe ocular and central nervous system damages. It is im¬ portant to know clinical, laboratory and radiological findings of this condition to apply effective antitoxic treatment and save the life and the vision of the pa¬ tient. Occasionally, decreased visual acuity, may be the only finding. In this study that was carried out iri three separate clinics for four months, the ocular and radiological findings of methyl alcohol intoxication cases, occasioned by consuming cologne as an alcoho¬ lic beverage, were reported. Seven of them ended in blindness, 2 of them died. Putamen necrosis was de¬ tected in 2 cases, edema of peripapillary nerve fibers was detected in 4 cases, both of which are findings of methanol intoxication. In cases in which electrophy- siological tests could have been applied, amplitudes and latency of a arid b waves were detected normal hi electroretinography, flash visual evoked potential waves were detected flat: antitoxic treatment methods were discussed emphasizing the role of ophthalmolo¬ gist in early diagnosis.
