Özet

Penetran keratoplasti sonrası astigmatizmanın azaltılmasında intraoperatif ve postoperatif sütür ayarlamasının etkinliği retrospektif olarak araştırıldı. Avasküler korneal patoloji nedeniyle penetran keratoplasti yapılan 73 "göz rastgele iki gruba ayrıldı. İntraoperatif sütür ayarlaması yapılan 31 göz birinci ve postoperatif sütür ayarlaması yapılan 42 göz ikinci grubu oluşturdu. İntraoperatif sütür ayarlaması asti-disk yardımıyla, postoperatif sütür ayarlaması postoperatif ilk 8 haftada 3.5 dioptri (D) ve üzerinde astigmatizma olan gözlerde biomikroskop altında yapıldı. Sütürler alınmadan önce ve sonraki dönemde iki grup arasındaki topografik astigmatizmaların karşılaştırılmasının istatistiksel analizleri yapıldı. Postoperatif ilk kontrolde intraoperatif sütür ayarlaması yapılanlarda 2.23±1.24 D olan ortalama astigmatizma, postoperatif sütür ayarlaması yapılmadan önce ikinci grupta 5.57±1.89 D bulundu (p=0.004). Postoperatif sütür ayarlaması sonrası ikinci gruptaki ortalama astigmatizma 2.06±1.23 D'ye düştü (p 0.05). İntraoperatif ve postoperatif sütür ayarlaması keratoplasti sonrası astigmatizmanın azaltılmasında etkili bulunmuştur. Ancak intraoperatif sütür ayarlaması yapılanlarda bu sonuçlar daha başarılı olmuştur.

Abstract

The efficiency of intraoperative and postoperative suture adjustment in reducing postkeratoplasty astigmatism is compared retrospectively. Seventy three patients undergoing penetrating keratoplasty for avascular comeal pathology were randomly assigned to two groups. The intraoperative suture adjustment was done with the help of an Asti-disk at the end of the operation. The postoperative suture adjustment was performed on eyes with postoperative astigmatism 3.5 Diopter (D) or more during the first postoperative 8 weeks. The topographic astigmatism of the two groups before and after suture removal was compared and analyzed statistically. By the first control of the group with intraoperative suture adjustment the mean astigmatism was 2.2311.24 D while 5.5?±1.89 D in the second group (p=0.004). After postoperative suture adjustment the mean astigmatism of the second group reduced to 2.06±1.23 D (p<0.001). The mean astigmatism of the first group was found less than the mean astigmatism of the second group, before and after suture removal but this .difference was not statistically significant (P>0.05). intraoperative and postoperative suture adjustment after penetrating keratoplasty is effective in reducing the postkeratoplasty astigmatism. Neverthless, the results of intraoperative suture adjustment are found better than postoperative suture adjustment.
