Posterior uvea malign melanomları (Tedavi özellikleri)

Özet
Posterior uvea melanomlannda uygulanacak tedavi şeklini belirlerken, etkilenen gözün görme keskinliği,basıncı, tümörün büyüklüğü, yayılımı ve yerleşimi, aktivitesi, diğer gözün durumu, hastanın genel sağlık ve psikolojik durumu dikkate alınmalıdır. Büyüme eğilimi göstermeyen küçük ve orta büyüklükteki tümörlerin bir bölümünde periyodik izlem, tedavi seçeneğidir. Büyüyen tümörlerde radyoterapi, lokal rezeksiyon veya fotokoagülasyon yapılabilir. İleri derecede görme kaybı olan büyük tümörlerde ise enükleasyon kaçmılmazdır.Bu çalışmada 1964-1993 yıllan arasında AÜTF Göz Hastalıkları ABD'da görülen 350 posterior uvea melanomlu olgunun tedavi yöntem ve sonuçları incelenmektedir. Tümörün boyutları ve hastalığın evresi gözönüne alınarak tedavi uygulanabilen 350 olgunun % 67,4'üne enükleasyon, % 18,6'sına preoperatif ışın + enükleasyon, % 2,6'sına enükleasyon + postoperatif ışın, % 4,3'üne Co plak, % 1,7'sine Co plak + enükleasyon, % 1,5'ine ise ışın tedavisi yapılmıştır. Olguların % 2,9'u tedavisiz izleme alınmıştır. Olgular ortalama 4 yıl izlenmişler ve bu süreçte düzenli izleme gelen 210 olguda yaşam oranı % 84,8 olarak saptanmıştır. Yaşayan ve kaybedilen olgularda sistemik metastaz görülme oranı ise % 9,5'tir. Erken tanı ve uygun tedavi ile metastazlara bağlı mortalite, uzun süreçte önemli bir sorun olmaktan çıkacaktır.
Abstract
Selecting a therapeutic approach in posterior uveal melanomas, certain factors such as the visual acuity .Intraocular pressure in the affected eye, the size of the melanoma, its extent and location, its apparent activity, the condition of the opposite eye, general health and psychological status of the patient must be carefully weighed. Periodic obsevation is believed to be the treatment of choice for most small and many medium sized melanomas that have not been documented to grow. If such lesions grow, then radiotheraphy, local resection or photocoagulation of the tumor can be used. In patients with ;e tumors that have produced severe visual loss, enucleation is inescapable. In this study, the treatment modalities and results of 350 posterior uvea melanoma pa¬tents seen in Ankara University Eye Clinic, during 1964-1993 were discussed.The size and extension of the tumor *We taken into consideration in order to determine the treatment modality. The choice of therapy was enucleation in 67.4 %, preoperative radiotherapy + enucleation % 18.6 %, enucleation + postoperative radiotherapy in 2,6 %, Co plaque therapy in 4.3 %, Co plaque + enucleation in 1.7 %, radiotherapy in 1.5 % of the 350 patients 2,9 % of. them were : observed periodically. The average follow-up period was 4 years and the survival rate was 84.8 % in 210 patients who were followed up during this period .The ratio of systemic metastasis was 9.5 %. It seems that, with early diagnosis and treatment, the mortality due to the disease will not be an important problem in the long term period.
