Özet
Retinoblastom (RB), çocukluk çağımn en sık görülen primer malign göz içi tümörüdür. 1963-1994 yıllan arasında Ankara Üniversitesi Tıp Fa¬kültesi Göz Hastalıkları Anabilim Dalı Oküler Onkoloji Bölümünde 636 olgunun 831 gözünde RB saptanmıştır. 636 olgunun 441'i (%69.3) tek taraflı, 195'i (%30.7) İse iki taraflıdır. Olguların 2681 (%42.1) kız, 368i (%57.9) erkektir. En küçük hasta 20 günlük, en yaşlı hasta ise 16 yaşındadır (ortalama 2.8 yıl). Otuzdört olguda (%5.3) ailede RB öyküsü saptanmıştır. Bu olgulardan 10'unda unilateral RB, 24'ünde ise bilateral RB mevcuttur. Onbir (%1.7) olguda spontan gerileme gözlenmiştir. RB'lu olguların kliniğe başvuru yakınmaları içinde birinci sırayı lökoko-ri (394 olgu, %61.9) almaktadır. Bunu gözde büyüme buftalmus (92 ol¬gu, %14.5) ve şaşılık (68 olgu, %10.7) izlemektedir. Hastaların refere edildikleri klinik ön tanı ile patoloji sonuçları 519 (%81.6) olguda birbi¬riyle uyumludur. Geri kalan 117 (%18.4) olguda klinik ön tam yalancı negatiftir. Bu olgulara kliniğimizde yapılan genel anestezi altında fun-dus muayenesi ve ultrason incelemeleri sonunda RB şühesi ağır bastığı için enükleasyon yapılmıştır. Karşılaşılan en sık ilk Uç yalancı negatif ön tamlar buftalmus (43 olgu, %6.8), endoftalmi (37 olgu, %5.8), ve re¬tina dekolmanıdır (12 olgu, %1.9). Belirtilen 636 olgunun dışında 29 ol¬guda ise yalancı pozitif RB tanısı konmuş ve gözler entiklee edilmiştir. Yalancı-pozitif tamlar: endoftalmi (9 olgu), retina displazisi (6 olgu), re¬tina dekolmam (5 olgu), vitreus kanaması (4 olgu), Coats hastalığı (4 ol¬gu) ve toksokariyazisdir (1 olgu). RB tanısı histopatolojik olarak ke-sinleştirildikten sonra tüm olgularda sistemik inceleme yapılmıştır. Göz içi RB olgularımızda kemik iliği dışında uzak organ ve santral sinir sis¬temi metastazı görülmemiştir. Göz içi evredeki olguların %6.1'inde ke¬mik iliğinde atipik hücre gözlenmiştir. Çocuklarda özellikle tek taraflı ve nedeni anlaşılamayan şaşılık, Uvelt-hipopiyon, endoftalmi, buftal¬mus, göz içi hemorajisi olgularında RB akla getirilmeli ve genel aneste¬zi altında fundus muayenesi; fundus aydınlanmıyorsa ultrason ve gerek görülürse bilgisayarlı tomografi ve manyetik rezonans görüntüleme is¬tenmelidir.
Abstract
Retinoblastoma (RB) is the most frequent malignant in¬traocular tumor in childhood. 636 cases with 831 affec¬ted eyes by RB have been diagnosed and treated by the Ocular Oncology department at the Ankara University Bye Clinic between 1963-1994. 441 (69.3%) cases were unilateral and 195 (30.7%) were bilateral. 268 (%42.1) were females and 368 (%57.9) were males. The youngest patient was 20 days old and the oldest was 16 years old at the time of diagnosis (mean: 2.8 years). In thirty-four cases (5.3%) a family history of RB was present. Ten of these cases were unilateral and 24 were bilateral. Sponta¬neous regression was noted İn eleven cases (1.7%). The most frequent presenting signs were leukokoria (394 cases, 61.9%), buphthalmos (92 cases, 14.5%) and strabis¬mus (68 cases, 10.7%). The results of the histopathologic examination showed that the referring initial diagnoses were correct in 519- cases (81.6%) and false-negative in 117 cases (18.4%). These latter eyes were enucleated be¬cause fundus and ultrasound examinations under general anesthesia strongly implied the presence of RB. The most frequent three initial false-negative diagnoses of the referring physicians were buphthalmos (43 cases, %6.8), endophthalmitls (37 cases, %5.8) and retinal detachment (12 cases, %1.9). Apart from these 636 cases, there were 29 false-positive diagnoses (4.4%). False positive dia¬gnoses were: endopthalmitis (9 cases), retinal dysplasia (6 cases), retinal detachment (5 cases), vitreous hemorrhage (4 cases), Coats disease (4 cases) and toxocarlasis (one case). All cases underwent complete review of the sys¬tems after the diagnosis of RB had been established. Aty¬pical malignant cells infiltrating the bone marrow were noted in 6.1% of the cases with intraocular RB. RB should be ruled out in cases of unexplained unilateral strabismus, uveitis-hypopyon, endophthalmltis, buphthal-mus and intraocular hemorrhage in children. Fundus exa¬mination under general anesthesia is mandatory. If effec¬tive fundus visualization is not possible, ultrasonography, computerized tomography and magnetic resonance ima¬ging techniques should be performed.
