Özet

Perforan göz yaralanmalarına bağh skarh kornealarda düzensiz kor¬nea yüzeyi ve genellikle eşlik eden afakiye bağh anizometropi ne¬deniyle gözlükle görme rehabilitasyonu mümkün olamamaktadır. A.Ü.T.F. Göz Hastalıkları Kliniği Kontakt Lens Servisi nda 33 post-travmatik skarh korneaya sert gaz geçirgen (SGG) kontakt lens uy¬gulanmıştır. Ylrmi-yedi (% 82) gözde, ortalama 19.3 aylık takipte komplikasyonsuz başanh bir uygulanım sağlanmıştır. On yaşından küçük olgularda başarı oranının (% 50), 20 yaşından büyük olgula¬ra göre (% 100) daha düşük olduğu gözlenmiştir. Başarı sağlana¬mayan 6 gözün 2'sinde neden ambliyopi ve yetersiz görme keskinliği iken, 2 gözde hastaların kontakt lens kullanma isteksizliği ve 2 gözde de kontakt lens intoleransı olmuştur. Kornea skannın yeri ve/veya büyüklüğü ile kontakt lens toleransı arasında belirli bir ilişki saptanamamıştır. Kontakt lense bağlı tek komplikasyon, 33 gözün 3 ünde (% 9) görülen punktan epitelyal keratit olarak belirlenmiştir. SGG kontakt lensler, özellikle erişkin yaş grubunda, posttravmatik skarh kornea olgulannda başarıyla uygulanmakta ve bu gözlerde kornea cerrahisine iyi bir alternatif oluşturmaktadırlar.

Abstract

Perforating corneal injuries often result in scarred corneas in which adequate visual rehabilitation can¬not be achieved with spectacles due to irregular cor¬neal surface. In these eyes, the presence of aphakia often adds to the problem with the coexlstance öf anisometropia. In this study, rigid gas permeable (RGP) contact lenses were fit in 33 post-traumatic scarred corneas of 33 patients. Twenty-seven of the 33 eyes (82 %) were successfully fit without complica¬tions for an average follow-up of 19.3 months. The success rate was observed to be lower (50 %) for pa¬tients less than 10 years old, whereas it was 100 % in patients older than 20 years. The six failures included two eyes with amblyopia and unsatisfactory visual acuity, two eyes in two patients who lacked motiva¬tion to use contact lenses, and two eyes that were contact lens intolerant. There was no significant cor¬relation between the failure groups and the location and size of the corneal: scar. The only contact lens related complication was punctate epithelial keratitis in three of the 33 eyes (9 %). The results of this stu¬dy indicate that RGP contact lenses are successful hi the majority of patiens (82 %) with post-traumatic scarred corneas, especially in the adult age group, and may obviate corneal surgery in these patients.
