Özet

57 glokom şüpheli ve 54 glokom tanısı konmuş toplam 111 gözün Humphrey otomatik statik perimetresi kullanılarak elde edilen görme alanlarının merkezi 30° ve periferik 30-50° nazal bölgeleri incelendi. Amaç ilave zamana gereksinim gösteren periferik görme alanı muayenesinin tanıya olan katkısının değerlendirilmesi idi.6 dB'in üstünde yanyana 2 ve/veya daha fazla, 10 dB'in üstünde 1 ve/veya daha fazla noktada hassasiyet azalmasının varlığı defekt kabul edildi.Gözlerin % 6.3'ünde (glokomlu 1 . glokom şüpheli 6 gözde) merkezi 30° de defekt görülmeksizin periferik nazal defekt bulundu.Merkezi 20 -30° nazal ve periferik 30 - 50° nazal izopterler incelendiğinde glokomlu 6 (%11.1), glokom şüpheli 13 (%22.8) gözde merkezi nazal bölge normal iken periferik nazal defekte rastlandı. Parasantral skotomlann yoğun bulunduğu arkuat sahalarda skotomlar oluşmadan önce periferik nazal defektin görülme sıkhğı glokomlularda %ll.l,glokom şüphelilerde %31.6 idi. Nazal bölgenin muayene süresi ortalama 3.95 dakika idi. Bu süre merkezi 30° testi süresini %25 uzattı. Sonuçta.özelllkle merkezde şüpheli skotomu olan hastalarda periferik nazal görme alanının tanıya yardımcı .hatta bazen tek başına tanı koydurucu değeri olduğu kanısındayız.

Abstract

A total of 111 eyes,57 glaucoma suspect and 54 glaücomatous.were examinated with automated Humphrey Visual Field Analyzer to evaluate the central 30° and peripheric nasal 30-50°. The purpose of the study was to find out if the time-consuming peripheric visual field examination was playing any role in the diagno¬sis. Any defect in the visual field was defined as;two arid/or more adjacent points with a decrease of more than 6 dB;one and/or more points with a decrease of more than 10 dB. 6.3% of eyes (one with glaucoma and 6 eyes with glaucoma suspect) had peripheric nasal visual field defect without any defect at the central 30° . When we considered 20-30° central nasal and 30-50° peripheric nasal isopters.in 6 (11.1 %)glaucoma patients and 13 (22.8%)glaucoma suspects .central nasal field was normal but defect was determined in the peripheric nasal field.The incidence of peripheric nasal defect before developing any paracentral scotoma in the arcuate area was 11.1% in glaucoma patients and 31.6% in glaucoma suspects. The time needed for the examination of the nasal field was mean 3.95 minutes.This means 25 % prolongation of the test time needed for the central 30°.As a conclusion especially in patients with suspected scotoma in the central field.peripheric nasal visual field examination may be diagnostic or add information for the correct diagnosis.
