Özet
Bu çalışmada kliniğimizde penetran keratoplasti+katarakt ekstrak-siyonu + göz İçi lens implantasyonunun simültane olarak uygulan¬dığı yaş ortalaması 42.5 (7-69) olan 15 olgunun, ortalama 24 ayhk (12-42 ay) takip sonuçları değerlendirilmiştir. Operasyon endikas-yonlannın çoğunluğunu grefon reddi (%33.3), komeal abse/keratit sekeli (%33.3) ve esîri perforan göz yaralanmasına bağh lökom (%20) oluşturmuştur. 6 olguda (%40) postoperatif 0.5 ve üzeri görme keskinliği sağlanmıştır. 13 olguda (%86.7) greft saydam kal¬mıştır. En sık rastlanan komplikasyonlar ön sinesi (%26.7), sekon-der katarakt (%26.7), kistoid makula ödemi (%13.3) ve glokom (%13.3) olmuştur. Grefon reddi gelişen iki olgudan birine rekerato-plasti uygulanmıştır. Üçlü girişimde uygulanan metod, kompli¬kasyonlar, avantaj ve dezavantajlar tartışılmıştır. Uygun olan vaka¬larda tercih edilecek bir yöntem olduğu düşünülmüştür.
Abstract
We have analysed the results of 15 combined proce¬dures of penetrating keratoplasty, extracapsular cata¬ract extraction and intraocular lens implantation bet¬ween November 1991 and November 1994. Indications for operation were graft rejection (33.3%), corneal ke-ratitis/abscess (33.3%) and leucoma due to previous perforating eye trauma (20%). After a mean postope¬rative followup of 24 months (12-42 months), 6 eyes (40%) achieved a visual acuity of 20/40 or better, 13 eyes (86.7%) had clear grafts. Most common complica¬tions were anterior synechia (26.7%), secondary cata¬ract (26.7%), cystoid macular edema (13.3%) and glau¬coma (13.3%). Of two eyes with graft rejection, one underwent rekeratoplasty. In the study, the methods, complications, advantages and disadvantages of triple procedure surgery were discussed.
