Tip II diabetes mellitusta diabetik retinopati: 2318 olguda ilk analiz

Özet
Tip II diabetli olgularda diabetik retinopatinin evrelerini, yaş, cinsiyet, diabet süresi, diabet tanı¬sının konduğu yaş, uygulanan diabet tedavisi, diabet dışında sistemik bir hastalık olup olmama durumunun diabetik retinopati ve evreleri üzerine etkilerini incelemek amacıyla tip II diabetes mellituslu 2318 olgunun ilk göz muayenesi verileri değerlendirildi. Olguların f203'ü erkek, 1115' i kadın olup ortalama yaş 57.47 idi. Bilinen diabet süresi en uzun 40 yıl olup, ortalama 11.74 yıldı. Diabetin tanı konduğu yaş ise ortalama 45.74 olarak saptandı. Olguların 1916'sı oral antidiyabetik, 371' i insülin kullanmaktaydı, 31' i ilaçsızdı. 2318 olgunun 20'sinde lens ve kornea kesafeti nedeniyle her iki gözde de diabetik retinopati değerlendirilemedi. 39 olguda diabetik retinopati yoktu. 225 olguda hafif nonproliferatif, 598 olguda anlamlı nonproliferatif, 248 olguda preproliferatif diabetik retinopati saptandı. 64 olguda yeni proliferatif değişiklikler olmaksızın laser nedbesi mevcuttu. 460 olguda proliferatif değişiklikler, 660 olguda ise ilerlemiş diabetik göz hastalığı izlendi. 4 olguda diabetik retinopatiye bağlı ftizis gelişmiş veya enükleasyon yapılmıştı. Sonuç olarak cinsiyetin diabetik retinopati evreleri üzerinde önemli bir etkisi yoktu. Diabet süre¬si, insülin bağımlılığı, ve diabetin tanı konduğu yaş ile diabetik retinopati evreleri arasında an¬lamlı ilişki saptandı.
Abstract
Diabetic Retinopathy In Type II Diabetes Mellitus:lnitial Analyse of 2318 cases In order to investigate the effect of the age, sex, duration of diabetes, age at diagnosis, treatment and other systemic disease on the phases of diabetic retinopathy, the results of the initial examination of 2318 cases with type II diabetes mellitus were evaluated. Of the patients, 1203 were male and 1115 female. The average age was 57.47 + 8.17. The duration of diabetes was maximum 40 years with the average being 11.74 + 6.99 years. The average age at diagnosis was 45-74 years. Both eyes of the 20 of 2318 cases could not be evaluated because of the opacification of the lens and cornea. Of the patients, 39 had no diabetic retinopathy, 225 had mild-moderate non-proliferative, 598 severe non-proliferative, and 248 preproliferative diabetic retinopathy. 64 cases had scars of photocoagulation without a new lesion. 460 cases had proliferative retinopathy while the remaining 660 had advanced diabetic eye disease. In 4 cases, the eyes were phthisic or enucleated secondary to complications of diabetic retinopathy. In conclusion, sex didn't effect the phases of diabetic retinopathy. But, there was a significant correlation between disease duration, insulin dependence, age at diagnosis and the phases of diabetic retinopathy.
