Özet

AMAÇ: Mini-nuk tekniği ile yapılan katarakt cerrahisinin hızlı görme rehabilitasyonu sağlamak için etkinliğini ve güvenilirliğini araştırdık.YÖNTEM: Toplam 54 hastanın 55 gözünde kullanılan teknik; üç adımlı skierai tünel keşi, kapsüloreksis, hidrodelineasyonun ardından ön kamara maintainer kullanılarak nükleus doğurtma, korteks aspirasyonu ve kapsül içi PMMA lensi yerleştirme şeklinde uygulandı.BULGULAR: Erken postop dönemdeki görme keskinliği ortalama 0.48+0.25 olup %50.9 oranında 0.5 ve üzerinde (arka segment problemli 2 olgu dışında %52.8 oranında 0.5 ve üzerinde) bulundu. Erken dönem astigmatizma ortalaması sütür konulan olgularda kurala uygun 0.85+0.35 D, sütür konmayan olgularda kurala aykırı 0.82+0.42 D idi. Tek önemli komplikasyon ilk 25 olgudan 18'inde (%72) görülen geçici kornea ödemi olup, yöntemin teknik detaylarının daha dikkatli uygulanması sonucu daha sonraki 30 vakanın sadece 2'sinde (%6.7) oluştu.SONUÇ: Mini-nuk tekniğinin küçük keşi ve düşük postoperatif astigmatizma nedeniyle erken postop dönemde çok iyi görme sonuçları sağlayan, güvenilir bir katarakt cerrahisi olduğu düşünüldü.

Abstract

PURPOSE: The safety and success rate of cataract surgery using mini-nuc technique in achieving early visual rehabilitation were evaluated. METHODS: The technique used in 55 eyes of 54 patients was composed of scierai tunnel incision (three step), capsulorhexis, hidrodissection/hidrodelineation, manual expression of nucleus, aspiration of cortex and implantation of an intraocular lens in the bag with the aid of anterior chamber maintainer. RESULTS: Mean visual acuity in the early postoperative period was 0.48+0.25 and was equal or better than 20/40 in 50.9% of eyes (52.8% when two eyes with retinal disease were excluded). Mean astigmatism was calculated to be 0.85+0.35 diopter with the rule in the sutured, versus 0.82+0.42 diopter against the rule in the suturless closures. The only significant complication, transient corneal edema was observed in 18 of the first 25 eyes (72%), but was reduced to 6.7% (in 2 of later 30 eyes) with thorough understanding of the details of surgical technique. CONCLUSION: Mini-nuc technique was considered to be a safe cataract extraction technique yielding excellent visual results because of small, tunnel type self-sealing incision with small amount of surgical induced astigmatism.
