İnferior oblik kas miyektomisi sonuçlarımız

Özet

AMAÇ: inferior oblik kas hiperfonksiyonunun tedavisinde, farklı başarı oranları ve komplikasyonları olan birçok cerrahi yöntem tarif edilmiştir. Bu çalışmanın amacı, inferior oblik hiperfonksiyonunun cerrahi tedavisinde miyektominin etkinliğini ve komplikasyonlarını değerlendirmektir. YÖNTEM: Haziran 1995 - Temmuz 1997 tarihleri arasında, inferior oblik hiperfonksiyonu sebebiyle insersiyonel miyektomi cerrahisi uygulanan 27 hastanın 36 gözü prospektif olarak değerlendirildi. Olguların 25' inde primer, 2' sinde ise sekonder inferior oblik hiperfonksiyonu mevcuttu. BULGULAR: Cerrahiyi takiben, 32 gözde (% 88.9) inferior oblik hiperfonksiyonu başarı ile düzeltildi, 4 gözde (% 11.1) ise rezidüel inferior oblik hiperfonksiyonu gelişti. Grade +1 rezidüel inferior oblik hiperfonksiyonu olan olgularımızın da başarılı kabul edilmesi ile cerrahi başarı % 97 olarak saptandı. Üç gözde (% 8.3) gelişen adherens sendromunun dışında başka bir komplikasyon izlenmedi. SONUÇ: inferior oblik kas miyektomisi, başarı oranı yüksek, hızlı uygulanabilir, etkili bir yöntemdir. Cerrahinin en önemli komplikasyonu adherens sendromudur.

Abstract

PURPOSE: Various surgical procedures were described for eliminating inferior oblique muscle overaction. The purpose of this study is to evaluate the success rate and complications of myectomy of the inferior oblique muscle. METHODS: Twenty-seven patients who had 36 myectomy of inferior oblique muscle at insertion between June 1995 to July 1997 were evaluated prospectively. Twenty-five patients had primary and 2 patients had secondary inferior oblique muscle overaction. RESULTS: Of 36 eyes, 32 eyes (% 88.9) had successful results and 4 eyes (% 11.1) had residual inferior oblique muscle overaction. With the grade +1 residual inferior oblique muscle overaction, the success rate was % 97. Except the 3 eyes (% 8.3) which developed adherence syndrome, no other complications were observed. CONCLUSION: The myectomy of the inferior oblique muscle is an effective procedure that possesses a high success rate, and easy performance. The most important complication of the surgery is adherence syndrome.
