Topikal betaxolol ve carteolol kullanılan vakalarda lipid profili

Özet
AMAÇ: Glokom tedavisinde kullanılan beta blokörlerin serum HDL kolesterol değerlerini düşürerek koroner kalp hastalığı riskini arttırdığı bildirilmektedir. HDL kolesterol düzeylerindeki düşüş intrinsik sempatomimetik aktiviteye sahip bir beta blokör olan Carteolol'ün topikal kullanımında daha az görülmektedir. Bu çalışmada intrinsik sempatomimetik aktiviteye sahip Carteolol ve intrinsik sempatomimetik aktiviteye sahip olmayan Betaxolol'un topikal kullanımda serum HDL kolesterol düzeyindeki etkilerinin farklı olup olmadığı araştırıldı. YÖNTEM: Yeni tanı konulan 22 primer açık açılı glokom (PAAG) vakası randomize olarak iki gruba bölünmüş ve ilk gruba topikal Carteolol, ikinci gruba topikal Betaxolol tedavisi 8 hafta süresince uygulanmıştır. BULGULAR: Tedavi sonrası alınan serum lipoprotein düzeyleri incelendiğinde Betaxolol kullanılan grupta serum HDL kolesterol düzeylerinin tedavi öncesi değerlere göre anlamlı ölçüde azaldığı, Carteolol kullanılan grupta ise istatistiksel olarak anlamlı bir değişiklik oluşmadığı gözlenmiştir. SONUÇ: HDL kolesterol seviyesindeki yükselmenin koroner arter hastalığı riskini azaltan önemli etkisi bilindiğinden bu özelliği nedeniyle risk taşıyan olgularda Carteolol'ün tercih edilmesi önerilebilir.
Abstract
PURPOSE: It has been shown that topical beta blockers used in the treatment of glaucoma increase the risk of coronary hearth disease by reducing the levels of serum HDL cholesterol. This effect, however, is less pronounced when intrinsic sympathomimetic activity effective Carteolol is used topically. In this study Carteolol which has intrinsic sympathomimetic activity and Betaxolol which hasn't, were compared about the effect of HDL cholesterol level on topical use. METHODS: In this study 22 newly diagnosed cases of primary open-angled glaucoma (PAAG) were randomly divided into two groups and received treatment for 8 weeks. The first group was treated with topical Carteolol and the second received topical Betaxolol. Before and following treatment with topical b-blockers, the levels of serum lipoprotein were established. RESULTS: It was observed that serum HDL cholesterol was at significantly lower levels after treatment with topical Betaxolol compared to pretreatment value. In the group treated with Carteolol, however, there was no significant change. CONCLUSION: Since it is known that HDL cholesterol has the important effect of reducing the risk of coronary-arterial disease, it may be advised that Carteolol should be preferred for use in high-risk cases.
