Özet

AMAÇ: Penetran keratoplasti sonrası travma sonucu greft ile alıcı arasındaki keşide ayrılma meydana gelen gözlerdeki sonuçları araştırmak. YÖNTEM: Penetran keratoplasti geçirmiş 540 olgudan postoperatif travma sonucu greft ile alıcı arasındaki keşide ayrılma meydana gelen 14 hastanın 14 (%2.6) gözündeki sonuçlar retrospektif olarak incelendi. Travma sonrası 14 olguda keşi yerinde ayrılmaya ilaveten 5 olguda lens luksasyonu (kaybı) ve 2 olguda göz içi lensinin keşi yerine dislokasyonu teşhis edildi. Disloke göz içi lensler çıkartıldı. Mevcut greftler tüm olgularda tekrar sütüre edildi. Afakinin düzeltilmesi amacıyla 1 olguda primer ve 2 olguda sekonder skierai fiksasyonlu arka kamara göz içi lensi (GİL) implantasyonu yapıldı. BULGULAR: Travma sonrası gözler ortalama 19.2 ay süre ile takip edildi. Son kontrolde 12(%85.7) olguda greftler saydam bulundu. 9(%64.2) olguda 0.5 ve daha iyi düzeltilmiş görme elde edildi. SONUÇ: Penetran keratoplasti sonrası travmatik keşi ayrılmasında ödemli de olsa mevcut greftin tekrar sütürasyonu ile greftler saydam kalmaktadır. Arka segment ile ilgili komplikasyon gelişmedikçe de görsel sonuçlar oldukça başarılı gözükmektedir.

Abstract

This study analyses the results of the eyes which had traumatic dehiscence of the surgical wound after penetrating keratoplasty. Of the 540 penetrating keratoplasty cases performed in our clinic, 14(2.6%) eyes were found to have traumatic separation between the donor and recipient interfaces following a blunt trauma. Of these cases, 5 patients had lens loss and 2 patients had intraocular lens dislocation to the wound. All these 7 patients had vitreus loss. Original corneal grafts were resutured in all cases. Scierai fixation IOL implantation was performed primarily in one case and secondarily in two cases. Mean follow-up period after trauma repair was 19.2 months. Corneal grafts were clear in 12 cases (85.7%). Best corrected visual acuity was better than 20/40 in 9 patients (64.2%). Our results suggest that even in the presence of marked edema, resuturing the original graft results clearing the cornea. Unless any compication of posterior segment progress, visual results seems to be favorable succesfull.
