Özet

AMAÇ: Çocukluk çağında penetran keratoplasti yapılan olgularda rektospektif değerlendirme ile greft prognozu ve komplikasyonlarının analizi yapıldı. GEREÇ VE YÖNTEM: Çocukluk çağında penetran keratoplasti yapılan 16'sı kız, 17'si erkek 33 olgunun 34 gözü retrospektif olarak değerlendirildi. BULGULAR: Olguların yaş ortalaması 6.42±6.01 yaş (3 ay-14yaş) idi. Penetran keratoplasti en sık lökom simpleksli (%54.1), keratokonuslu (%17.8) ve konjenital endotelyal distrofi (%17.8) olgulara uygulandı. Olguların ortalama 18.35±11.65 ay (4-36 ay) takiplerinde greftlerin %73.5'unun saydam olduğu ve %26.5'inde greft yetmezliği geliştiği gözlendi. Greft yetmezliği gelişen olguların %66.7'si konjenital endotelyal distrofili ve vaskülarize olgulardı. Görme keskinliği değerlendirilen 28 gözün 18'inde 1/10 ve üstünde, 10'nunda 5 metreden parmak sayma ve altında görme seviyesi tespit edildi. SONUÇ: Ambliopiyi önlemek ve greft saydamlığını sağlamak için çocuklarda erken dönemde keratoplasti yapılmalı ve sık aralarla izlenmelidir.

Abstract

We evaluated 34 eyes of 33 cases who underwent penetrating keratoplasty in childhood. The study group consisted of 16 female, 17 male with a mean age of 6.42±6.01. We performed keratoplasty mostly to cases with leucoma simplex (54.1%), keratoconus (17.8%), congenital endothelial dystrophy (17.8%). The follow-up period ranged from 4 to 36 months (mean 18.35±11.65). Graft was transparent in 73.5% of the cases whereas graft failure developed in 26.5%. Graft failure developed in cases with congenital endothelial dystrophy and vascularized cornea. The evaluation of visual outcome revealed visual acuity over 1/10 in 18, less than 1/10 in 10 cases.We can suggest that in children, keratoplasty should be performed in nearly terms and the patients should be frequently followed up in order to obtain clear graft and to prevent ambliopia
