Penetran keratoplasti ile kombine göz içi lens yerleştirilmesi

Özet

AMAÇ: Kliniğimizde Ocak 1992-Aralık 1996 tarihleri arasında, korneal opasite ile birlikte kataraktı olan veya psödofakik, afakik bülloz keratopatisi mevcut 62 olgunun, 62 gözüne uygulanan kombine cerrahi sonuçları değerlendirildi. GEREÇ VE YÖNTEM: 4 olguya Penetran Keratoplasti ve Ekstrakapsüler Katarakt Ekstraksiyonu (PK-EKKE), 1 olguya Penetran Keratoplasti ve Ekstra Kapsüller Katarakt Ekstraksiyonu Ön Kamara Göz İçi Lensi İmplantasyonu (PK+EKKE+ÖKGİL), 6 olguya Penetran Keratoplasti ve Sekonder Arka Kamara Göz İçi Lensi İmplantasyonu (PK+Sekonder AKGİL), 24 olguya Penetran Keratoplasti ve Sekonder Skierai tespitli Göz İçi Lensi İmplantasyonu (PK-Sekonder Skierai tespitli GİL) uygulanmıştır. Hastalar en az 6 ay, en fazla 4 yıl süreyle izlenmiştir. BULGULAR VE SONUÇLAR: Ameliyat sonrası komplikasyon olarak; 8 grefon reddi, 11 geçici göz içi basıncı artması, 3 glokom, 2 grefon yetmezliği, 2 sütür açılması, 2 koroid dekolmanı, 2 GİL desantralizasyonu görülmüştür.Takip süresi içerisinde grefon saydamlık oranı %83.9, 0.1 ve üzeri görme keskinliği oranı %56 olarak bulunmuştur.

Abstract

PURPOSE: We evaluated combined surgical procedures; which are performed on 62 patients whose eyes have either corneal opacity with cataract or pseudophakic/aphakic bullous keratopathy between January 1992 and December 1996 in our clinic. SUBJECT AND METHODS: Among those patients, penetrating keratoplasty (PK) with extracapsular cataract extraction (ECCE) are performed on 4 patients; PK with ECCE and anterior chamber intraocular lens (ACIOL) implantation were performed on one patient; PK with secondary posterior chamber IOL implantation are performed an six patients, PK with secondary scierai fixated IOL implantation are performed on 24 patients. Follow-up periods were ronging between 6 months and 4 years. RESULT: Among postoperative complication there were 8 graft rejections, 11 transient increasein intraocular pressure, 3 glaucomas, 2 graft failures, 2 suture failures, 3 choroidal detachments, 2 IOL malpositions. In the followup period, graft clarity rate is found 83.9% and visual acuity that is above 2/20 is found 56%.
