Özet
Korneadaki yüzeyel opasiteler ve düzensizliklerin excimer laser kullanılarak tedavi edilmesine fototerapötik keratektomi (PTK) adı verilmektedir. Bu tedavi öncesinde yüzeyel korneal opasiteleri ve düzensizlikleri lameller veya penetran keratoplasti ile tedavi edilmekteydi. Bu çalışmada korneal opasite veya düzensizliği bulunan 12 hastanın 15 gözüne FTK uygulanmış ve hastalar ortalama 13.4 ay süreyle takip edilmiştir. Hastaların düzeltilmiş görme seviyeleri 2 gözde sabit kalırken, 13 gözde görme keskinliği artmıştır. Postoperatif dönemde tüm hastalarda hipermetropik kayma izlenmiş, ancak uygulanan geniş zonlu ablasyon tekniği ile anizometropi oluşumu engellenmiştir. Postoperatif dönemde açığa çıkan subepiteiyai korneal bulanıklık hafif derecede izlenmiş ve preoperatif dönemle kıyaslandığında ilave skarlaşmaya neden olmadığı gözlenmiştir. > Uygun olgular seçildiği takdirde son derece başarılı sonuçların elde edildiği, bu tedavi şekli korneal transplantasyona kıyasla daha az invazif bir yöntemdir.
Abstract
Phototherapeutic Keratectomy Phototherapeutic keratectomy (PTK) is a new technique that removes corneal opacities and smoothens irregular corneal surfaces by using excimer laser energy. Belon FTK, the treatment of corneal opacition and irregularition was heratoplasty. PTK was used to treat 15 eyes of 12 patients with corneal opacities and followed up average of 13.4 months. Best spectacle corrected visual acuity did not change in 2 eyes, and improved in 13 eyes. Hyperopic shift was observed in all patients in the postoperative period, but the large ablation zone with the corneal shaping mode decreased the hyperopic shift and anizometropia was not observed in these patients. Postoperative corneal haze was graded as no more than mild in all cases and additional scarring compared to the preoperative level was not occured. The result of PTK is very successful in selected patients and this treatment modelity seems to be less invasive with respect to corneal transplantation.
