Cerrahi deneyimin fakoemülsifikasyon sonuçlarına etkisi

Ocak 1995-97 tarihleri arasında fakoemiilsifikasyon ve göz içi lens implantasyonu yapılan 402 hastanın 500 gözü cerrahın deneyimine göre 4 gruba ayrıldı. 3 cerrahın ilk 50 olgusu l.grubu oluşturdu. II. III. ve IV. grup aynı cerrahların 2.3. ve 4.50 olgusu ile oluşturuldu. Bütün grupların görme sonuçları ve komplikasyonları araştırıldı ve cerrahi deneyimin fakoemülsifikasyon sonuçlarına etkisi analiz edildi. Hastaların ortalama yaşı 62.4±11.3 (14 90) idi. Takip süresi 2 ay ile 20 ay arasında değişmekteydi. Ameliyat sırasındaki komplikasyon oranı l. grupta %37.5, II. grupta %23.3, III. grupta %18.0 ve IV. grupta %14.0 olarak bulundu (p 0.05). Fakoemülsifikasyona başlangıç ve geçiş döneminde komplikasyon insidansı yüksek olabilir. Cerrahın fakoemülsifikasyon cihazına ve tekniğine henüz tam olarak alışmadığı erken dönemde daha yumuşak kataraktlı olguların seçilmesi ve fakoemülsifikasyon deneyimi olan merkezlerde dikkatli bir eğitim komplikasyon oranının en aza indirilmesinde yararlı olabilir. Fakat buna rağmen fakoemüfsifikasyonda peroperatuvar ve postoperatuvar komplikasyon oranının cerrahi deneyim ile yakın ilişkisi vardır. Cerrahi deneyim arttıkça peroperatuvar ve postoperatuvar komplikasyonlar belirgin biçimde azalmaktadır.
Abstract
Five hundred eyes of 402 cases who underwent phacoemulsification and intraocular lens implantation between January 95 - 97 were divided into four groups according to the experience of the surgeons. The first 50 cases of 3 surgeons consisted the first group. The second, third and forth group consisted of the second 50, the third 50, and the forth 50 patients of the same surgeons. We evaluated the visual outcome and complications of the groups and analysed the effect of surgical experience on phacoemulsification results. The mean age of the patients were 62.4±11.3 years (range 14 and 90). The follow up period ranged from 2 to 20 months. The intraoperative complication rate was found to be 37.5% in the first, 23.3% in the second, 18% in the third, and 14% in the forth groups respectively (p<0.05). Best corrected visual acuity of 5/10 or more was observed in 89.4%, 90%, 88%, and 90% in the group I, II, 111, and IV respectively (p>0.05). The incidence of complication can be high at the initial and transition period. At the early period surgeons who are not used to phacoemulsification machine and technique should select cases with soft cataract and practicing in experienced clinics can be useful in reducing the complication rate. However, intraoperative and postoperative complication rate is closely related to the experience of the surgeon. The decrease in intraoperative and postoperative complications will be evident with surgical skill gained gradually.
