Göz yorgunluğu kuru gözün önemli bir semptomu mudur?

Özet
AMAÇ: Çalışmamızda, göz yorgunluğu şikayetinin kuru gözle ilişkisinin ve öneminin araştırılması planlanmıştır. YÖNTEM: Göz polikliniğimize başvuran 124'ü erkek, 130'u bayan olarak 254 kişi (yaş ortalaması 49.2±10.7) göz yorgunluğu ve/veya kuru göz varlığı yönünden değerlendirildi. Kuru göz tanısı, semptomların sorgulanması, rose bengal ve flöresein boyama testleri, gözyaşı kırılma zamanı (GKZ) ölçümü ve Schirmer testi yapılarak kondu. BULGULAR: 254 kişinin 69'u (%27.2) göz yorgunluğu tarif ederken 41'inde (%16.1) kuru göz tesbit edilmiştir. Semptomların kuru gözle ilişkisine bakıldığında, en kuvvetli ilişkinin göz yorgunluğu ile olduğu bulundu (p<0.001). Göz yorgunluğundan şikayetçi 69 hastanın 26'sında (% 37.7) kuru göz vardı ve tüm grupta kuru göz bulunma oranı olan % 16.1' den fazla idi (p<0.001). Ayrıca, 41 kuru göz hastasının 26'sında (% 63.4) göz yorgunluğu şikayetleri varken, bu tüm hastalardaki göz yorgunluğu oranından (% 27.2) belirgin olarak fazla idi (p<0.001). Kuru göz hastalarında şikayet sayısı kontrollere göre daha fazla idi. Kuru gözde 5.0± 2.9, kontrolde 1.1.±1.0, p<0.001). SONUÇ: Göz yorgunluğu ve kuru göz arasında kuvvetli bir ilişki olup, kuru göz muayenesinde göz yorgunluğunun sorgulanması ve göz yorgunluğunun ayırıcı tanısında kuru gözün dikkate alınması gerektiğini düşünmekteyiz.
Abstract
PURPOSE: To establish the relation between ocular fatigue and dry eye. METHODS: 254 consecutive patients were evaluated for the presence of ocular fatigue and/or dry eye (124 male, 130 female; mean age:49.2± 10.7 y.o.). In the diagnosis of dry eye, the presence of symptoms and the scores of rose bengal staining, fluorescein staining, break-up time measurement and the Schirmer test were evaluated. RESULTS: Among all the patients, 69 (27.2%) reported symptoms of ocular fatigue and 41(16.1%) were diagnosed as dry eye. Of the 69 patients complained of ocular fatigue, 26 (37.7%) had dry eye which was significantly higher than the 16.1% for the entire group (p<0.001). 26 of all dry eye patients (63.4%) complained of ocular fatigue, which was also significantly higher than the 27.2% among all patients (p<0.001). Dry eye patients had greater number of complaints (5.0+2.9 per patient) compared to controls(1.1±1.0 per patient)(p<0.001). CONCLUSION: These findings suggest that there is a strong relationship between ocular fatigue and dry eye. The questionare of ocular fatigue in the diagnosis of dry eye, and the consideration of dry eye in the differential diagnosis of ocular fatigue seem to be important.
