Özet

Diabetik hastada retinada belirgin morfolojik değişiklikler oluşmadan önce nörosensoriyal fonksiyon değişikliklerinin oluştuğu bilinmektedir.Bilgisayarlı görme alanı muayenesi de bu değişiklikleri saptamada kullanılan yöntemlerden biridir. Biz çalışmamızda retinopatisiz ve çok hafif retinopatili Tip II diabetli 43 hastanın 74 gözüne Octopus otomatik périmètre ile $G_1$ analizi yaptık. Aynı yaş grubundan diabeti olmayan 9 kişinin 18 gözü kontrol grubu olarak alındı. Hastalarımızda diabet hastalığının süresi önemli olmaksızın hepsinde kontrol grubuna oranla istatistiksel olarak anlamlı Mean Defekt artışı yani genel hassasiyet azalması saptanırken, diabet süresi 5 yılın üzerindeki hastalarda ayrıca lokal defektleri gösteren Loss Variance'da da kontrol grubuna oranla istatistiksel olarak anlamlı artış saptandı. Santral $10^\circ$'nin görme alanının en fazla etkilenen kısmı olduğu belirlendi. Bu değişiklikler belirgin retinopatinin gelişeceğinin habercisi olarak düşünüldü.

Abstract

In the diabetic patient, neurosensorial functional alterations are known to be occurred prior to the development of significant morphological changes. Automated visual field testing is one of the examination techniques used to detect them. In our study, $G_1$ global analysis of Octopus automated perimetry was performed in 74 eyes of 43 type II diabetic patients with minimal or no retinopathy. 18 eyes of 9 healthy subjects in the same age were elected as a control group. Mean defect, the indicator of generalized depression, was found to be increased in diabetics when compared with the controls. This increase was independent of the duration of diabetes, while loss variance, the indicator of localized depression, was increased only in the diabetic patients in whom the duration of type II diabetes was 5 years or longer. Central $10^\circ$ was found to be the most effected part of the visual field. These alterations were considered to be the precursor of clinically apparent diabetic retinopathy.
