Özet
AMAÇ: Primer (blefarofimozis sendromu, konjenital ankiloblefaron) ve sekonder (travmatik) telekantuslu olgularda uygulanan güncel cerrahi yaklaşımları ve bu durumlara eşlik edebilecek lakrimal drenaj sistemi deformasyonun ve ptozisin cerrahi tedavisini vurgulamaktır. YÖNTEM: Primer telekantusu bulunan blefarofimozis sendromlu (BS) 5 olguda telekantusun düzeltilmesi için Mustarde'nin çift Z plastisi uygulandı. Konjenital ankiloblefaronlu 2 olguda ise, iç kantoplasti ile telekantus düzeltildi. Travmatik telekantuslu 5 olgunun üçünde latérale yer değiştirmiş orbital dokular transnazal telleme ile karşı taraf sağlam medial kantal tendona tutturularak telekantus giderildi. Aynı amaçla diğer 2 olguda ise medial kantal dokular orbita medial duvarına miniplak implantlar ile sütüre edildi. Travmatik telekantuslu 4 hastadaki lakrimal sistem deformasyonu ve konjenital ankiloblefaronlu bir olgunun iki gözündeki lakrimal sistem agenezisi, konjonktival dakriosistorinostomi ile düzeltildi. BS'lu olgulardaki blefaropitozis ile travmatik telekantuslu bir olgudaki ptozis fasia lata askılama yöntemi ile giderildi. BULGULAR: Primer telekantuslu 7 olguda 11-58 (ortalama 34) ay, travmatik telekantuslu 5 olguda 10-40 (ortalama 25) ay izleme sonucu, tatminkar fizyolojik ve kozmetik başarı elde edildi. Hastaların hiçbirinde ciddi komplikasyon ile karşılaşılmazken iki olguda ilk operasyonda yerleştirilmiş olan Lester-Jones tüplerinin değiştirilmesi gerekti. SONUÇ: Uygulanan özel cerrahi yöntemlerle primer ve sekonder telekantusta başarılı sonuçlar elde edilmektedir.
Abstract
PURPOSE: To discuss surgical options in the management of primary (blepharophimosis syndrome, congenital ankyloblepharon) and secondary (traumatic) telecanthus with accompanying ptosis and lacrimal drainage system deformity.METHOD: Mustarde's double Z-plasty was performed in 5 cases with blepharophimosis syndrome (BS) and medial canthoplasty was performed in 2 cases with congenital ankyloblepharon demonstrating primary telecanthus. In traumatic telecanthus, laterally displaced orbital tissues were attached to the medial canthal tendon of the intact side by transnasal wiring technique in 3 of 5 cases, and to miniplates that were screwed to the medial orbital wall in the other 2 cases. Lacrimal system deformity in 4 traumatic telecanthus cases and bilateral lacrimal drainage system agenesis in 1 congenital ankyloblepharon case were corrected with conjunctiva! dacriocystorhinostomy. Ptosis in 5 BS and in 1 traumatic telecanthus was corrected with fascia lata suspension technique. RESULTS: Seven cases with primary telecanthus were followed up for 11-58 (average 34) months and 5 cases with traumatic telecanthus were followed up 10-40 (average 25) months with satisfactory physiologic and cosmetic results. No serious complications were recorded. CONCLUSION: Satisfactory results were achived in primary and secondary telecanthus cases with specific surgical techniques.
