Özet

AMAÇ: Üstten ve temporalden "saydam clear korneal" girişli fakoemülsifikasyon sonrası oluşan astigmatik değişikliklerin değerlendirilmesi. METOD: Bu çalışmada korneada yapılan insizyon yerine göre üstten ve temporalden giriş olarak iki grup oluşturuldu. Bütün hastalara fakoemülsifikasyon yöntemi ile lens ekstraksiyonunu takiben katlanabilir göz içi lensi implante edildi. Üstten giriş grubu 18 hastanın 34 gözünden, temporalden giriş grubu 16 hastanın 22 gözünden oluşuyordu. Bütün hastalara preoperatif, postoperatif 1. gün, 2. hafta, Lay, 3. ay ve 6. ay keratometrik ölçümleri yapıldı. Cerrahinin neden olduğu astigmatizm vektör analizi kullanılarak değerlendirildi. BULGULAR: Saat 12'den giriş yapılan ilk grupta postoperatif 1. ay sonunda vektör 1.50 ile 2.90 arasında ortalama 2.07 (Std 0.348), aksı 121, 6. ay sonunda vektör 0.42 ile 2.30 arasında ortalama 1.47 (Std 0.411) aksı 107 derece olarak bulundu. Temporal giriş yapılan ikinci grupta postoperatif vektör 0.72 ile 1.60 arasında 0.94 (Std 0.258) aksı 168 derece olarak bulundu. Her iki grubun vektörel değerleri arasındaki fark istatistiksel olarak anlamlı idi. (p<0.05) YORUM: Çalışmamızda temporal girişin üstten girişe göre daha az postoperatif astigmatizmaya yol açtığı ve daha erken vizüel ve refraktif stabilizasyon sağladığı görüldü. Aynı zamanda çalışmamızda üstten girişin kurala aykırı, temporal girişin ise kurala uygun astigmat indüklediği tesbit edildi.

Abstract

PURPOSE: To evaluate the astigmatic difference between superior and temporal clear corneal incisions for phacoemulsification. METHOD: In this study patients of two group were formed according to their location of incisions of clear cornea, one group of which was superior and the other was temporally applied. All patients were implanted foldable lOL's after lens extractions by phacoemulsification method. 34 eyes of 18 patients were superior incisions group and 22 eyes of 16 patients were temporal incision group. We performed keratometric measurements of all patients at periods preoperatively, postoperatively 1. day, 1. week, 1. month, 3. month and 6. month. Surgically induced astigmatism were studied by vector analysis method. RESULTS: In superior entrance group has a vector of between 1.50 and 2.90 mean of 2.07 (std 0.348) on axis 121 at 1st month and 0.42 and 2.30 mean of 1.47 (std 0.411) on axis at 6 month. In temporal entrance group has a vector of 0.78 and 1.90 mean of 1.11 (std 0.326) on of 107° axis 157 at 1st month and between 0.72 and 1.60 mean of 0.94 (std 0.258) on axis 168 at 6 months. The difference of vectorel values between the two groups were statistically significant (p<0.05).DISCUSSION: In our study temporal incision causes less astigmatism than 12 o'clock incisions and this causes refractive stabilisation and early visual acuity. At the same time, we have found that superior entrance induced without rule astigmatism, temporal entrance with rule astigmatism.
