Özet

Bu çalışmada maksilla etmoid dekompresyonunda Caldwell-Luc yaklaşımına karşılık.transnazal endoskopik yaklaşımın etkinliği ve morbiditési araştırılmıştır. Hasta sayısı henüz az olmakla birlikte, maksilla-etmoid dekompresyonu için endoskopik transnazal yöntem ile Caldwell-Luc yaklaşımının serbestleştirilen hacimle uyumlu olarak benzer etkinlik sağladığı görülmüştür. Endoskopik yaklaşımla medyal duvarda daha fazla yer kazamlabildiği için dışa bakış kısıtlılığı ve horizontal düzlemde kayma ortaya çıkabilirken, her iki yöntemde de simetrik olmayan cerrahide vertikal düzlemde kayma oluşabilmektedir. Erken dönem cerrahi morbidité açısından ise endoskopik yöntemin daha iyi tolère edildiği gözlenmiştir.

Abstract

In this study, we investigated efficacy and surgical morbidity of transnasal endoscopie maxilla-ethmoid decompression which is an alternative of the Caldwell-Luc approach in patients who present with an indication of surgical decompression for thyroid ophthalmopathy. Both techniques proved to be effective in controlling active infiltrative phase of the disease. Although there are relatively small number of cases in each group transnasal endoscopie method has been shown to have equal efficacy with the Caldwell-Luc approach, related with the decompressed volume of the orbita. On the occasion of excessive medial orbital decompression with the endoscopie approach, limited abduction and ocular deviations in horizontal plane can occur. In cases of asymmetric decompression of medial and inferior orbital walls, vertical deviation can arise in both methods. Endoscopie approach seemed to have less early postoperative morbidity.
