Lens veya göz içi mercek dislokasyonlarında pars plana vitrektomi ve sıvı perflorokarbon kullanımı

Özet
AMAÇ: Bu çalışmada lens veya göziçi mercek (GİM) dislokasyonu nedeniyle pars plana vitrektomi yapılan olgularda sıvı perflorokarbon (perflorofenantren, perflorodekalin) kullanımı ve bu konudaki deneyimlerimiz irdelenmiştir. YÖNTEM: Lens veya GİM dislokasyonu olan 15 olgunun 15 gözüne pars plana vitrektomi, sıvı perflorokarbon (perflorofenantren, perflorodekalin) yardımı ile güvenli lens ekstraksiyonu yapıldı.8 olguya ayrıca skleral fiksasyonlu arka kamara lens implantasyonu yapıldı. Ortalama takip süresi 10 aydı (1-30 ay). BULGULAR: Hiçbir olguda intraoperatif komplikasyon görülmedi. Ortalama 10 aylık izlem sonunda 10 olguda (%67) görme artışı saptandı. 2 olguda (%13) görme değişmedi, 3 olguda (%20) görmede azalma saptandı. Postoperatif dönemde 2 olguda retina dekolmanı, 2 olguda kistoid makula ödemi, 2 olguda hipotoni gözlendi. SONUÇ: Disloke lens veya GİM'lerin çıkarılmasında pars plana vitrektomi sonrası perfluorokarbon sıvı kullanımının cerrahi travmayı azaltıcı, retinayı koruyucu ve bu sebeple intraoperatif ve postoperatif komplikasyonları azaltıcı, güvenli tampon maddeler olduğu sonucuna varılmıştır.
Abstract
PURPOSE: To review the use of perfluorocarbon liquids in the removal of lens or intraocular lens dislocation and our own experiences. METHODS: We used pars plana vitrectomy and perfluorocarbon liquids (perfluorophenanthrene, perfluorodecalin) to facilitate the safe removal of 15 posteriorly dislocated lenses or intraocular lenses. After removal of dislocated lenses scleral fixated posterior chamber lenses were implanted in 8 patients. Mean follow up time was 10 months. RESULTS: No intraoperative complication observed. After mean follow up of 10 months visual acuity improved in 10 (67%}, decreased in 3 (20%) and remained the some in 2 (13%) patients. During the follow-up period, we observed retinal detachment in 2 patients, cystoid macular edema in 2 patients and hypotony in 2 patients. CONCLUSION: In the removal of posteriorly dislocated lenses or intraocular lenses we can suggest that combination of pars plana vitrecromy with perfluorocarbon liquids will have a beneficial effect on intraoperative and postoperative complications of surgery and safe removal of lenses.
