İntrakameral lidokain anestezisi ile fakoemülsifikasyon cerrahisi

Özet
AMAÇ: Fakoemülsifikasyon cerrahisinde topikale ilave intrakameral prezervan içermeyen lidokain anestezisinin etkinliğini ve güvenilirliğini araştırmak. YÖNTEM: 120 hastanın 133 gözüne topikale ilave %1'lik intrakameral prezervan içermeyen lidokain anestezisi ile saydam korneal kesiyle fakoemülsifikasyon yapıldı. BULGULAR: Fakoemülsifikasyon sırasında 6 (%4.5) gözde arka kapsül perforasyonu meydana geldi. Bu gözlerde ön vitrektomi sonrası sulkusa göz içi lens (GİL) im plantasyonu, diğer 127 olguda kapsül içine GİL implantasyonu yapıldı. Ön kameranın prezervan içermeyen lidokain ile irrigasyonu sırasında 2 olguda geçici ışık hissi kaybı dışında anesteziye bağlı bir komplikasyon tespit edilmedi. SONUÇ: Topikale ilave intrakameral prezervan içermeyen lidokain anestezisinin fakoemülsifikasyon ve gelişen komplikasyonların tedavisi için etkili ve güvenli bir anestezi sağladığı gözlendi.
Abstract
PURPOSE: To investigate the effectiveness and safety of intracameral preservative-free lidocain administration in addition to topical anesthesia in phacoemulsification surgery. METHOD: Intracameral 1% preservative-free lidocaine anesthesia was combined with topical anesthesia in 133 eyes of 120 patients who underwent phacoemulsification with clear corneal tunnel. RESULTS: In 6 cases (4.5%) posterior capsule rupture occured, anterior vitrectomy was performed and IOL was implanted to the sulcus. In 127 cases with no peroperative complication in the bag IOL implantation was performed. All patients well tolerated the surgery and no additional anesthesia was required even in complicated cases. Two patients (1.5%) reported transient loss of light perception. CONCLUSION: Our results with intracameral preservative-free lidocaine as a supplement to topical anesthesia suggest its effectiveness and safety in phacoemulsification surgery and even if the surgery is complicated.
