Orbita blow-out kırıklarında cerrahi tedavi yaklaşımları

Özet
AMAÇ: Orbita blow-out kırıklarında uygulanan tedavi yöntemleri ve kliniğimizde uygulanan cerrahi sonuçlarının değerlendirilmesi. YÖNTEM: Orbita tabanında blow-out kırığı tanısı alan ve travma sonrası ilk iki hafta içinde düzelmeyen ciddi enoftalmi ve/veya diplopi nedeniyle cerrahi tedavi uygulanan 9 olgu incelenmiştir. Tüm olgularda lateral kantal insizyonu takiben alt konjunktiva forniksinden yapılan kesiyle orbita tabanına ulaşılmıştır. Defekt bölgesi, 3 olguda otojen kemik grefti ile, 4 olguda Polietilen implant ile ve 2 olguda ise silikon blok ile kapatılmıştır. Dört olguda, Caldwell-Luc girişimi de cerrahiye eklenmiştir. BULGULAR: Hastalar 2-13 ay süreyle takip edilmiştir. İki olgu dışında tüm hastalarda diplopinin düzeldiği görülmüştür. Diplopinin sebat ettiği iki olguda ise inferior rektusa uygulanan geriletme cerrahisi ile düzelme sağlanmıştır. Tüm olgularda enoftalmi ve hipoftalmide belirgin düzelme sağlanmıştır. SONUÇ: Travma sonrası erken dönemde düzelmeyen enoftalmi, hipoftalmi ve/veya diplopi olan blow-out kırıklarının cerrahi tedavi sonuçları tatminkar ve komplikasyon yönünden güvenlidir.
Abstract
PURPOSE: To evaluate the surgical treatment modalities and the results of surgical applications in orbital blow-out fractures in our clinic. METHOD: Nine cases of blow-out fractures with persistent severe enophthalmos, and/or diplopia two weeks after trauma were treated surgically. Lateral cantholysis with inferior conjunctival incision approach was preferred for all cases. To repair floor defects we used autogenous bone grafts in 3 cases, polyethylene implants in 4 cases and silicon block in 2 cases. Caldwell-Luc surgery was also applied in 4 cases. RESULTS: Patients were followed for 2 to 13 months. All but 2 cases of diplopia were corrected with blow-out surgical repair. Diplopia correction was achieved after recession of inferior rectus muscle in these two cases. Enophthalmos and hypophthalmos were corrected sufficiently in all cases. CONCLUSIONS: Results of surgical treatment in orbital blow-out fractures with persistent enophthalmos, hypophthalmos and/or diplopia are satisfactory and safe for complications.
