Ketorolak trometamin %0.5, disodyum kromoglikat %2 ve lodoksamid trometamin %0.1' in vernal keratokonjonktivit tedavisindeki etkinliklerinin karşılaştırılması

AMAÇ: Vernal keratokonjonktivit (VK) hastalarında, ketorolak trometamin %0.5 (ketorolak) ile mast hücresi membran stabilizatorleri disodyum kromoglikat %2 (DSKG) ve lodoksamid trometamin %0.1'in (lodoksamid) tedavi etkiniliğinin karşılaştırılması. YÖNTEM: Üç gruba ayrılan aktif dönemdeki 33 vernal konjonktivit hastasına, topikal ketorolak, DSKG yada todoksamid tedavisi uygulandı. VK'in temel belirtileri (kaşıntı, sulanma, çapaklanma, yanma-batma, fotofobi) ve bulguları (konjonktival hiperemi, papiller hipertrofi, papiller flöresein boyanması, kornea epitel defekti, limbal infiltratlar) 0., 7., 14. ve 21. günlerde değerlendirilerek, ilaçların tedavi etkinliklerinin karşılaştırılması amaçlandı. BULGULAR: Ortalama bulgu ve belirti skorlarında, ketorolak ve lodoksamid gruplarında 7.günden, DSKG grubunda ise 14. günden itibaren anlamlı azalma olduğu izlendi (p 0.05). SONUÇ: VK'in bulgu ve belirtilerini azaltmadaki etkinlikleri açısından ketorolak, lodoksamid ve DSKG arasında fark olmamasına rağmen, DSKG'ın etkisinin ketorolak ve lodoksamide göre daha geç ortaya çıktığı izlendi.
Abstract
PURPOSE: To compare the efficacy of ketorolac tromethamine 0.5% (ketorolac) with mast cell membrane stabilizers, disodium cromoglycate 2% (DSCG) and lodoxamide tromethamine 0.1% (lodoxamide) in the treatment of vernal keratoconjunctivitis (VK). METHOD: Thirty three patients with active vernal conjunctivitis were divided into three groups and received either ketorolac, DSCG or lodoxamide for a period of 21 days. Principal signs (itching, lacrimation, mucus discharge, stinging sensation, photophobia) and symptoms (hyperemia, papillary hypertrophy, flourescein staining of papilla, corneal epitheliopathy, limbal infiltrates) of VK were evaluated at baseline, 7th, 14th and 21st days in order to compare the efficacies of the three study drugs. RESULTS: Ketorolac and lodoxamide reduced the mean sign and symptom scores of VK significantly at the 7th day of the study, on the contrary, DSCG reduced mean sign and symptom scores after the 14th day of the study (p<0.05). Although ketorolac and lodoxamide was significantly superior to DSCG in reducing the signs and symptoms of VK in the first 7 days of the study, there was no statistically significant difference among the three drugs after the 14th day of the study (p>0.05). CONCLUSION: Although three drugs used in the study were effective in reducing the signs and symptoms of VK ketorolac and lodoxamide reduced the signs and symptoms in a shorter period of time when compared to DSCG.
