Özet
AMAÇ: Malignite nedeniyle yapılan total orbital ekzenterasyonlardan sonra ortaya çıkan geniş orbital bölge defektlerini temporal kas flebi kullanarak rekonstrüksiyon yaptığımız olgularımızın sonuçlarının değerlendirilmesi. YÖNTEM: Bu çalışmada orbita ve çevresine yayılım göstermiş olan malign tümörlü 14 olguda total ekzenterasyon ve geniş tümör eksizyonu uygulandı ve oluşan doku defekti aynı seans da çevrilen temporal kas flebi ve flebin üzerine örtülen uyluk iç yüzünden alınan kısmi kalınlıkta deri grefti ile onarıldı. BULGULAR: Postoperatif olguların hiç birinde flepde beslenme bozukluğu görülmedi, önemli bir komplikasyona rastlanmadı. 7-56 ay arasında, ortalama 28 aylık bir izlem sonucunda olguların hiç birinde rekürrens ve metastaz saptanmadı. SONUÇ: Ekzenterasyon sonrası oluşan orbital bölge defektlerinin rekonstrüksiyon unda, temporal kas ve fasianın bölgeye komşuluğu nedeni ile rekonstrüksiyon için operasyon süresini fazla uzatmaması, donör sahada ikincil bir deformite yaratmaması, daha sonra yerleştirilebilecek protez için uygun bir yatak oluşturması ve kozmetik olarak kabul edilebilir bir sonuç sağlaması nedeni ile seçilebilecek iyi bir cerrahi yöntem olduğu sonucuna varıldı.
Abstract
PURPOSE: To evaluate the results of the cases in which temporal muscle flaps are used in reconstruction of the large orbital area defects those occur after total orbital exenterations because of malignancies. METHODS: In this study, after total exenteration and large tumor excision of 14 cases of malignant tumors that invased orbits and surrounding areas, tissue defects are reconstructed with temporal muscle flaps and partial width skin grefts taken from the internal side of thigh to cover the flaps at the same session with the exenteration operation. FINDINGS: Postoperatively none of the cases showed circulatory disturbances of flap and had significant complications. in the 7-56 months (mean 28 months) follow up time, in none of the cases recurrences or metastases are recorded. RESULTS: Large orbital area defects after total orbital exenterations can be reconstructed with temporal muscle and fascia successfully, because; being adjacent to the operation area, operation time for reconstruction is not so long, no secondary deformities occurred at the donor area and besides an acceptable cosmetic result, a suitable socket for the proteses that can be placed afterwards can be achieved with this method.
