Glokomlu olgularda latanoprost'un aköz PGF2alfa ve metaboliti düzeyleri üzerine etkisi

Özet
AMAÇ: Bu çalışmada, glokomlu gözlerde uygulanan latanoprostun aköz hümör PGF 2α ve metaboliti düzeyleri üzerine olan etkisinin araştırılması planlanmıştır. GEREÇ VE YÖNTEM: Trabekülektomi planlanan, primer açık açılı veya kapsüler glokomlu olgular, Grup 1 (kontrol grubu) (n=17) ve Grup 2 (latanoprost grubu) (n=9) olarak ikiye ayrılmıştır. Preoperatuar olarak 10 gün boyunca topikal ilaçlar kesilmiş, gerekli olan olgularda sistemik karbonik anhidraz inhibitörü kullanımına devam edilmiştir. Grup 2'de olgulara preoperatuar olarak 5-10 gün boyunca topikal tek doz %0.005 latanoprost uygulanmıştır. Trabekülektomi sırasında parasentez ile tüm olgulardan aköz hümör örneği alınmış ve enzim immünoessey ile PGF 2α ve metaboliti düzeylerine bakılmıştır. Bulgular: ortalama PGF 2α düzeyi kontrol grubunda 24.38±5.79 pg/ml ve latanoprost grubunda 10.99±4.11 pg/ml'dir ve aradaki fark istatistiksel olarak anlamlı bulunmuştur (p 2α ile metaboliti düzeyleri arasında pozitif bir korelasyon mevcuttur (p 2α analoğu olan latanoprost'un operasyon öncesi topikal kullanımı, aköz hümör PGF 2α düzeyini muhtemelen artmış uveoskleral akım dolayısıyla düşürmüştür. Bu etkinin anti-enflamatuar yararları olabileceği düşünülmüştür.
Abstract
OBJECTIVE: The purpose of this study is to evaluate the effect of latanoprost on the aqueous humor PGF<sub>2α</sub> and metabolite levels in glaucoma patients. MATERIALS AND METHODS: Patients diagnosed with either capsular or primary open-angle glaucoma and scheduled for trabeculectomy, were enrolled in the study. Group 1 represented the control group (n=17) and Group 2 represented the latanoprost treatment group (n=9). All topical drugs were stopped 10 days preoperatively and only systemic carbonic anhydrase inhibitors were continued if necessary. Group 2 patients received topical 0.005% latanoprost once daily for 5-10 days preoperatively. During trabeculectomy operation, aqueous samples were taken with paracentesis from alt of the patients. Levels of PGF<sub>2α</sub> and its metabolite 13,14-dihydro-15-keto-PGF<sub>2α</sub> in the aqueous humor were measured using enzyme-immunoassay. RESULTS: The mean PGF<sub>2α</sub> levels were 24.38±5.79 pg/ml in the control group and were 10.99±4.11 pg/ml in the latanoprost group, the difference of which was statistically significant (p<0.05). in the latanoprost group, there was a positive correlation between levels of PGF<sub>2α</sub>, and its metabolite (p<0.05). CONCLUSION: Pretreatment with PGF<sub>2α</sub> isopropyl ester analogue, latanoprost topically decreased PGF<sub>2α</sub> levels in the aqueous humor, probably due to its uveoscleral outflow increasing effect, exerting an anti-inflammatory- like effect.
