Prematür retinopatisi sekeli olarak psödoekzotropya ile birlikte ezotropya

Özet

Gözlerde dışa kayma şikayetiyle başvuran altı yaşındaki kız çocuğunun muayenesinde, psödoekzodeviasyon görünümüne rağmen açma kapama testinde manifest ezodeviasyon tespit edilmiştir. Prematür doğum öyküsü olan hastanın fundus bakısında bilateral temporal maküler ektopi ile beraber diskte ve retinal damarlarda temporal çekinti, periferik temporal vitrede kondansasyon saptanmıştır.Prematür retinopatisinin sekeli olarak karşımıza çıkan maküler ektopi, psödostrabismusa yol açabilmektedir. Psödostrabismus hem var olan manifest şaşılığı etkileyebilmekte hem de hekimin dikkatini direkt olarak şaşılığın tedavisine çekerek, altta yatan nedeni gizlemektedir. Bu nedenle tüm psödostrabismus vakalarının tam bir şaşılık muayenesinden geçirilmesi, dikkatli bir hikaye alınması, iyi bir fundus bakısı yapılması büyük önem taşımaktadır.

Abstract

Despite the inspection of pseudoexotropia, we had diagnosed manifest esodeviation in the cover-uncover test of a six year-old girl who had the complaint of exodeviation. There were bilateral temporal macular ectopia, temporal dragging of the papilla and the retinal vessels and temporal peripheral vitreal condansation in the fundoscopic examination of the patient who also had the history of prematurity. Macular ectopia, as a sequela of retinopathy of prematurity, could cause pseudostrabismus. Pseudostrabismus might affect preexisting manifest deviation and also hide the underlying cause of the strabismus by taking the attention of the physician onto the treatment. Therefore, it is very important to get a good history to perform a complete strabismus examination and a fundoscopy in cases of pseudostrabismus.
