Topikal anestezi ile fakoemülsifikasyon: İlk sonuçlarımız

Özet
AMAÇ: Topikal anestezinin, hasta rahatlığı ve cerrahi komplikasyonlar açısından güvenirliğini değerlendirmek. GEREÇ VE YÖNTEM: Ellialtı katarakt hastasının 62 gözüne topikal anestezi ile fakoemülsifikasyon ve göz içi lens implantasyonu uygulandı. Topikal anestezi için %0.5'lik bupivakain hidroklorür emdirilmiş cerrahi sponclar, alt ve üst fornikslere uygulandı. BULGULAR: Postoperatif 1. günde 40 hastanın (%65) düzeltilmemiş görme keskinlikleri 5/10'un üzerindeyken, 1.ayda 6 hasta (%10) dışında tüm hastaların düzeltilmiş görme keskinlikleri 5/10'un üzerindeydi. İlk 16 vakada, topikal anesteziyi takiben korneal epitelyal ödem ve abrazyon gözlendi. Hiçbir hasta, topikal anestezi sırasında ağrı duymadı. Görsel ağrı skalasına göre, ameliyat sırasında 8 hasta (%13) çok hafif, 22 hasta (%36) hafif, 18 hasta (%29) orta, 10 hasta (%16) şiddetli, 4 hasta (%6) dayanılmaz ağrı duyduğunu ifade etti, Göz hareketleri, hiçbir vakada ameliyatın seyrini etkileyecek düzeyde rahatsızlık vermedi. SONUÇ: Deneyim kazanıldıkça, topikal anestezi ile fakoemülsifikasyon ve göz içi lens implantasyonu, hasta ve cerrah açısından rahat ve güvenli bir yöntem olarak kabul edildi.
Abstract
PURPOSE: To evaluate the reliability of the topical anesthesia for the patient comfort and surgical complications. MATERIALS AND METHODS: Phacoemulsification followed by intraocular lens implantation by topical anesthesia was performed to 62 eyes of 56 patients to evaluate for topical anesthesia, surgical sponges soaked with bupivacaine hydrochloride (0.5%) were applied to the fornices. RESULTS: On the first postoperative day, the uncorrected visual acuity of 40 patients (65%) was 5/10 or better. On the first postoperative month, the corrected visual acuity was 5/10 or better in all of the patients except for 6 cases (10%). Corneal epithelial edema and abrasion after the topical anesthesic agent administration were seen in the first 16 cases. None of the patients described pain during the administration of the topical anesthetics. According to the visual pain scale, 8 patients (13%) described mild discomfort, 22 patients (36%) slight, 18 patients (29%) moderate, 10 patients (16%) severe, 4 patients (6%) unbearable pain during the surgery. Eye movements did not affect the surgical plan in the patients. CONCLUSION: Phacoemulsification followed by IOL implantation by topical anesthesia can be accepted as a reliable method for the patient and surgeon as long as the surgeon gets skilled.
