Özet

AMAÇ: Uzun süreli fluktuasyon (USF), farklı görme alanı muayenelerinde ölçülen retinal duyarlılık değerlerinin değişkenliğini ifade eder. Bu çalışmanın amacı, oküler hipertansiyonlu gözlerin görme alanında USF'un miktarını ve ilişkili olduğu etkenleri belirlemektir. YÖNTEM: Oküler hipertansiyonlu 19 hastanın 32 gözüne ait 127 adet görme alanı muayenesi incelendi. Her göz için en az 3 görme alanı vardı ve muayeneler arasındaki ortalama süre, 8.9 aydı. Humphrey Field Analyzer'ın 30-2 tam eşikdeğer programıyla elde edilen görme alanlarının tümü, cihazın ölçütlerine göre güvenilirdi. BULGULAR: Retinal duyarlılık, test noktalarının %96'sında 20 desibelin (dB) üstündeydi, ortalama olarak 27.7 dB'di. Ortalama USF, standart sapma olarak 2.6 dB, değer aralığı olarak 5.4 dB'di. Fluktuasyon miktarı retinal duyarlılık ile ilişkiliydi, duyarlılık azaldığında USF artıyordu (r=0.44, p<0.0001). Görme alanının periferinde ve üst yarıalanda USF miktarı daha fazlaydı. Ortalama yanlış-negatif yanıt oranı, test süresi ve soru sayısı değerlerinden herbiri ile USF arasında anlamlı bir bağıntı vardı (r=0.39 P<0.05; r=0.38, p<0.01 ve r=0.45, p<0.01). SONUÇ: Oküler hipertansiyonda eşikdeğer değişkenliği, retinal duyarlılığa ve test noktalarının lokalizasyonuna bağlıdır. Fluktuasyonu etkileyebilecek diğer etkenler, yanlış-negatif yanıt oranı, test süresi ve soru sayısıdır. Görme alanları yorumlanırken, bu etkenlerin göz önünde tutulması yararlı olacaktır.

Abstract

PURPOSE: Long term fluctuation (LTF) means the variability of retinal sensitivity values measured in different visual field examinations. The aim of this study is to determine the magnitude of LTF and the factors related to LTF in the visual field of eyes with ocular hypertension. METHOD: We studied 127 visual field examinations of 32 eyes of 19 patients with ocular hypertension. There were at least three visual field s for each eye and the mean duration between the tests was 8.9 months. All of the tests were obtained by Humphrey Field Analyzer using full threshold central 30-2 program. The tests were reliable according to the criteria of the producer. Mean sensitivity and LTF were calculated for each test location. RESULTS: Retinal sensitivity was higher than 20 decibel (dB) in 96 % of the test locations, mean value was 27.7 dB. The average LTF was 2.6 dB as measured by standard deviation and 5.4 dB as measured by range. The amplitude of fluctuation was correlated with retinal sensitivity; the fluctuation showed an increase when the sensitivity was low (r=0.44, p<0.0001). The value of LTF was greater in the peripheries and the superior hemifield of the visual field. There was a significant correlation between the LTF and each of the following: the mean of false-negative response rate, the test time, and the number of questions (r=0.39 p<0.05; r=0.38, p<0.01 and r=0.45, p<0.01). CONCLUSION: Threshold variability was dependent on retinal sensitivity and test point location in ocular hypertension. Other factors that potentially may influence threshold fluctuation include false-negative response rate, test time, and number of questions. To take these factors into account may facilitate interpretation of visual fields.
