Fakoemülsifikasyonda korneal insizyon boyutunun cerrahi astigmatizmaya etkisi

Özet
AMAÇ: Fakoemülsifikasyonla katarakt cerrahisinde, sütürsüz kornea kesilerinin boyutunun, cerrahi astigmatizmaya etkisinin değerlendirilmesi amaçlanmıştır. GEREÇ VE YÖNTEM: 32 göze 4 mm superior şeffaf korneal tünel insizyonu ve 24 göze 5.2 mm superior şeffaf korneal tünel insizyonu ile fakoemülsifıkasyon katarakt cerrahisi yapıldı. Ameliyat öncesinde çalışmaya alınan tüm gözlerin keratometrik ölçümleri alındı ve bu ölçümler ameliyat sonrası 2.gün, 1.hafta, 2.ay, 4.ayda tekrarlandı. Cerrahi sonucu gelişen astigmatizme miktarı ve astigmatik aks değişikliği vektör analiz metodu ile değerlendirildi. BULGULAR: Cerrahi astigmatizma açısından iki insizyon grubunun vektör analizi ile karşılaştırılması sonucunda 5.2 mm korneal insizyon grubunda daha fazla cerrahi astigmatizma oluşmasına rağmen postoperatif 2.gün, 1.hafta, 2. ay ve 4.ayda istatistiksel anlamlı fark gözlenmemektedir (sırasıyla p=0.36, p=0.35, p=0.43, p=0.63). Vektör analiz yöntemiyle her iki grup cerrahi astigmatizma aksı yönünden karşılaştırıldığında; postoperatif 2.gün, 1 .hafta, 2.ay ve 4.aylarda anlamlı fark bulunmamıştır (sırasıyla p=0.76, p=0.41, p=1.0, p=0.43). Her iki grupta da cerrahi astigmatizmanın vektör analizi yöntemiyle değerlendirilmesinde, vektör aksları 0-45 ve 135-180 dereceleri arası kümelenmektedir. Cerrahi astigmatizma aksları arası anlamlı fark olmaması ve aksların açısal dağılım şekli her iki grubun da kural dışı astigmatizmaya yol açtığını göstermektedir. SONUÇ: Sonuç olarak 4 mm ve 5.2 mm sütürsüz şeffaf korneal insizyonlu katarakt cerrahisinde postoperatif gelişen astigmatizma ve kural dışı astigmatizma miktarları anlamlı fark oluşturmamaktadır. Küçük korneal tünel insizyonun herhangi bir nedenle genişletilmesi gereken durumlarda, katlanabilir lens implantasyonunun mümkün olmadığı vakalarda 5.2 mm insizyonun 4mm insizyona tercih edilebileceğini ve postoperatif aynı derecede iyi sonuçlar alınabileceğini düşünmekteyiz.
Abstract
PURPOSE: To evaluate the effects of no stich corneal incisions with different sizes on surgically induced astigmatism in phacoemulsificatiort cataract surgery MATERIALS AND METHODS: 32 eyes had 4 mm and 24 eyes had 5.2 mm superior clear corneal tunnel incisions for phacoemulsification cataract surgery. Keratometric measurements were taken preoperatively and repeated at 2 days, 1 week, 2 months, 4 months postoperatively from all eyes. Surgically induced astigmatism and astigmatism axis change were analysed by using vector analysis method. RESULTS: There was no statistically significant difference by comparing the two groups for surgically induced astigmatism by using vector analysis at 2 days, 1 week, 2 months, and 4 months postoperatively (p=0.36, p=0.35, p=0.43, p=0.63 respectively), although 5.2 mm corneal incision caused more surgically induced astigmatism. No significant difference between the two groups was obtained by using vector analysis for surgically induced astigmatism axis change at 2 days, 1 week, 2 months, 4 months postoperatively (p=0.76, p=0.41, p=1.0, p=0.43 respectively). The cumulation of astigmatic axis degree values were observed between 0-45 and 135-180 degrees in both groups by using vector analysis. Astigmatic axis distribution pattern and no statistically significant difference between astigmatic axis degrees in two groups show that both corneal incisions cause against the rule astigmatism. CONCLUSION: There was no statistically significant difference between 4 mm and 5.2 mm no stich clear corneal incision cataract surgery for the surgically induced astigmatism and against the rule astigmatism. In cases which corneal small incision enlargement becomes necessary and foldable lens implantation is not appropriate, 5.2 mm incision can be choosen over 4 mm incision and the same satisfactory outcome can be obtained with 5.2 mm incision postoperatively.
