Yüksek riskli proliferatif diabetik retinopati hastalarında panretinal fotokoagülasyon

Özet
AMAÇ: Bu çalışmada, yüksek risk özellikleri taşıyan proliferatif diabetik retinopatili (PDR) olgularda iki yıllık bir izleme süresinde (ortalama 23.3 ay) panretinal fotokoagülasyon tedavisinin etkinliği değerlendirildi. YÖNTEM: Toplam 73 hastanın 107 gözü incelendi. Bu gözlerden 80'inde sadece PDR, 27'sinde hem PDR hem de hipertansif retinopati (HR) mevcuttu. 41 gözde disk neovaskülarizasyonu (DNV), 41 gözde retinal neovaskülarizasyon (RNV) ve 25 gözde DNV+RNV tesbit edildi. 25 gözde bu bulgulara ek olarak vitreus hemorajisi vardı. BULGULAR: Panretinal fotokoagülasyon tedavisinden sonra, 55 gözde görme değişmezken (%51.4), 19 gözde artma (%17.8) ve 33 gözde azalma (%30.9) belirlendi. Bu gözlerin 16'sında (%15) şiddetli görme kaybı gelişti. FFA bulguları da görme sonuçlan ile uyumlu değişiklikler gösterdi. Vitreus hemorajisi bulunması önemli bir risk faktörü oluşturmaktaydı. Bu olgularda şiddetli görme kaybı riski, hemorajisi olmayanlara göre 3 kattan fazla bulundu. SONUÇLAR: Elde edilen bulgular, yüksek risk karakterleri taşıyan PDR'li hastalarda 2.000-2.500 şutla yapılan başlangıç tedavinin, daha fazla spotla tedavi öneren çalışmalarla benzer özellikler taşıdığını göstermektedir.
Abstract
PURPOSE: This study was designed to see the effects of panretinal photocoagulation on proliferative diabetic retinopathy (PDR) with high-risk Characteristics during the two years of a follow-up period;. (mean 23.3 months). METHODS: One hundred and seven eyes of 73 patients were examined. Eighty eyes had PDR, and 27 with both PDR and hypertensive retinopathy (HR). Fortyone eyes with neovascularisation of the disc (NVD), 41 eyes with neovascularisation of retina (NVE) and 25 eyes with NVD+NVE were; studied. There were vitreous haemorrhage in 25 eyes. ' RESULTS: After panretinal photocoagulation, no visual change was observed in 55 eyes (51.4%), an increase of vision was observed in 19 eyes (17.8%) and a decrease of vision was observed in 33 eyes (30.9%). The 16 of these eyes had severe visual loss (15%). FFA changes were similar to these results. Vitreous haemorrhage was also an important risk factor. The risk of severe visual loss in these patients were found more than three times as to the patients without haemorrhage. CONCLUSIONS: These data obtained from the panretinal photocoagulation of PDR with high risk Characteristics using 2000 -2500 spots were similar to other studies using a more intensive photocoagulation treatment at the beginning.
